PTEN和cyclin E在胃癌中的表达及其临床意义

目的]研究PTEN和cyclin E在胃癌组织中的表达水平以及与生物学行为的关系和对预后评价的意义。方法]以免疫组织化学方法检测106例胃癌组织中PTEN和cyclin E蛋白的表达水平。结果]106例胃癌中,PTEN表达阳性72例(67.9%),其中弱阳性15例,强阳性57例;cyclin E蛋白阳性52例(49.1%),其中弱阳性30例,强阳性22例。在胃癌组织中,PTEN蛋白水平与胃壁浸润深度、组织分化程度,临床pTNM分期相关(P<0.05):cyclin E蛋白表达水平与胃壁浸润深度、组织分化程度、临床pTNM分期相关(P<0.05);PTEN与cyclin E蛋白阳性表达显著相关(P<0.05);单变量生存分析,cyclin E蛋白阳性表达组3年、5年生存率分别为25%,15.4%,阴性表达组为55.6%,42.6%,二者有显著差异(P<0.05)。COX回归模型多变量分析显示,PTEN和cyclin E均不是独立的预后指标。结论]PTEN和cyclin E均与胃癌的发生发展有关,二者在胃癌表达中存在负相关性,其中cyclin E对胃癌预后判断具有一定的价值。

EXPRESSIONS OF PTEN AND CYCLIN E IN GASTRIC CARCINOMA AND THEIR CLINICAL IMPLICATIONS

Objective] To study the expressions of PTEN and cyclin E in gastric cancerous tissues and their relationships with biological behavior and to discuss their significances for prognostic evaluation.Methods] The expressions of PTEN and cyclin E in gastric cancerous tissues were detected in 106 cases of gastric carcinoma by the immunohistochemical method.Results] Among the 106 cases,72 cases(67.9%) had positive expression of PTEN with 15 cases of mild expression and 57 cases of strong expression;52 cases(49.1%) demonstrated positive expression of cyclin E with 30 cases of mild expression and 22 cases of strong expression.In gastric cancerous tissues,both the expressions of PTEN and cyclin E were associated with the invasion depth of gastric wall,degree of histological differentiation and clinical stage of PTNM(P<0.05);PTEN was significantly related with the positive expression of cyclin E(P<0.05).By univariate analysis,the 3year and 5-year survival rates in cases with positive expression of cyclin E were 25% and 15.4%,respectively,while those in cases with negative expression were 55.6% and 42.36%,respectively;the difference was statistically significant(p<0.05).The result of COX multivariate regression analysis suggested that neither PTEN nor cyclin E was independent prognostic indicator.Conclusions] Both PTEN and cyclin E are linked to the development and progression of gastric carcinoma.Their expressions in gastric cancerous tissues are of negative relation.Cyclin E has a certain value in the prognostic evaluation of gastric carcinoma.
