带状疱疹后遗神经痛危险因素Logistic回归分析

目的 探讨带状疱疹后遗神经痛（postherpetic neuralgia,PHN）的危险因素及更有效的治疗手段.方法 对469名带状疱疹患者进行问卷调查,其中遗留后遗神经痛患者66例,对可能与后遗神经痛有关的危险因素进行 Logistic回归分析,筛选影响后遗神经痛的危险因素.结果 带状疱疹后遗神经痛发生比例为14.072%（66/469）.经Logistic回归分析表明：年龄对应的Exp（B）=OR=1.050＞1,P=0.000,有统计学意义（P＜0.05）;治疗前VAS（Visual analogue scales,VAS）评分对应的Exp（B）=OR=1.024＞1,P=0.027,有统计学意义（P＜0.05）,针灸治疗对应的Exp（B）=OR=4.882＞1,P=0.000,有统计学意义（P＜0.05）.结论 年龄和治疗前VAS评分是PHN的危险因素,对于年龄较高,疼痛较明显的患者应及早接受规范诊治;针灸疗法是其保护性因素,使用抗病毒药物同时可联合针灸治疗,以提高临床疗效.

Logistic Regression Analysis of Risk Factors in Post-herpetic Neuralgia

Objective To discuss the risk factors and the better effective therapeutic approach in the treatment of post - herpetic neuralgia（PHN）. Methods Four hundred and sixty - nine patients of her- pes zoster were in the questionnaire investigation. Sixty - six patients of them had PHN and were involved in Logistic regression analysis so as to screen the related risk factors of PHN. Results The percentage of PHN morbidity was 14.072% （ 66/469 ）. The Logistic regression analysis indicated ; corresponding to age, Exp （ B ） = OR = 1. 050 〉 1, P = 0.000, indicating the statistical signifieance（ P 〈 0.05 ） ; before treatment, correspond- ing to VAS score, Exp（B） = OR = 1. 024 〉 1, P = 0. 027, indicting the statistical significance （ P 〈 0. 05 ） ; corresponding to the treatment of acupuncture and moxibustion, Exp （B） = OR = 4. 882 〉 1, P = 0. 000, indi- cating the statistical significance （ P 〈 0.05 ）. Conclusion The age and VAS score before treatment are the risk factors of PHN. For those of older age and with quite remarkable pain, the regular diagnosis and treatment should be applied earlier. The therapy of acupuncture and moxibustion is the protective factor and it can be used in combination with the anti -viral drugs to improve the clinical efficacy.
