异丙酚复合芬太尼靶控静脉麻醉与静吸复合麻醉临床比较

摘    要:目的:比较异丙酚复合芬太尼靶控静脉麻醉与静吸复合麻醉的各项临床指标。方法:ASAⅠ、Ⅱ级择期行腹腔镜胆囊切除手术患者90例,随机分为静吸复合组(C组)和异丙酚复合芬太尼靶控组(F组),每组各45例。观察麻醉诱导、气管插管、苏醒过程及术毕停药后患者各项指标,并加以比较。结果:①C组麻醉诱导时低血压者多于F组,插管反应发生率高于F组。②两组患者术后自主呼吸恢复时间、呼之睁眼时间、拔管时间差异无统计学意义(P〉0.05),但F组的定向力恢复时间和离开术后恢复室(PACU)的时间早于C组。两组患者术后恶心、呕吐发生率,差异无统计学意义(P〉0.05)。结论:与常规静吸复合麻醉下行腹腔镜胆囊切除术比较,异丙酚复合芬太尼靶控静脉麻醉诱导更加平稳,在苏醒质量上更优良。

Abstract:Objective:Comparison of propofol target-controlled intravenous fentanyl combined with inhalation anesthesia and the clinical indicators. Methods:ASA Ⅰ,Ⅱ elective laparoscopic cholecystectomy patients,90 patients were randomly divided into combined inhalation group (group C) and propofol-fentanyl TCI group (group F) 45 cases. Observed induction of anesthesia,tracheal intubation,recovery process and after treatment of surgery patients and compared the indicators. Results:①Group C during induction of anesthesia hypotension than group F,intubation was higher than in the F group. ②two groups of postoperative recovery time of spontaneous breathing,respiratory,eye opening,extubation time,the differ ence was not statistically significant (P〉0.05),but the F group of orientation recovery time and leave the recovery room (PACU) time early in the group C. F group of patients left the PACU immediately after extubation,the extubation,1 h and 3 h after the OAAS score higher than the group C,two groups of patients the incidence of postoperative nausea and vomiting was no significant difference (P〉0.05). Conclusion:Combined anesthesia with conventional laparoscopic cholecystec-tomy when compared to target controlled infusion of propofol induction of anesthesia,fentanyl is more stable and better quality in the wake.
