内镜下治疗胃肠道巨大脂肪瘤的临床研究

目的探讨内镜下切除胃肠道巨大脂肪瘤（瘤体直径＞3.0 cm）的安全性和有效性.方法近10年在我院经内镜下切除15例有症状的胃肠道巨大脂肪瘤患者,5例亚蒂型（基底直径＜2.0cm）中2例经由息肉切除术切除,3例采用多步切除术切除;10例为广基型（基底部直径＞2.0 cm）,经由次全切除术切除.结果内镜下成功切除15例胃肠道巨大脂肪瘤（经组织病理学确诊）,无穿孔和出血等严重并发症,内镜检查随访1～8年无复发.结论胃肠道巨大脂肪瘤可在内镜下由圈套器安全有效地切除.

Clinical study on endoscopic therapy of giant gastrointestinal lipomas

Objective To investigate the safety and feasibility for endoscopic removal of giant gastrointestinal lipomas (>3. 0 cm in diameter). Methods During last 10 years, fifteen giant and symptomatic gastrointestinal lipomas were resected under endoscopy in our hospital. Of them, two giant lipomas with small stalk (< 2. 0 cm in basal diameter) were resected by polypectomy; ten giant lipomas with base size greater than 2 cm in diameter were removed by a "subtotal resection". Other three giant lipomas with small talk were resected by multi-step resection. Results All 15 lesions were successfully removed endoscopically, and histopathologically confirmed to be lipomas. No severe complications such as perforation and hemorrhage were occurs after endoscopic removal. No recurrence was observed during 1-8 years endoscopic follow-up. Conclusion Giant gastrointestine lipomas can be removed safely by elec-trosurgical snare resection under endoscopy following the guidance of the present therapeutic strategy.
