肺结核病人血清中IL-22和IL-17的表达水平及临床意义分析

摘    要:目的研究肺结核病人血清IL-22和IL-17的表达水平并分析其临床意义。方法采用ELISA法检测59例肺结核患者(TB)和44例健康志愿者(HD)外周血清IL-22和IL-17的表达水平,同时检测了白细胞计数(WBC)、红细胞沉降率(ESR)、C反应蛋白(CRP)的水平,并分析细胞因子之间及细胞因子与三项炎症指标的相关性。结果 TB组外周血清IL-22和IL-17的浓度分别为26.70±10.63pg/ml和128.02±6.96pg/ml,均显著高于HD组IL-22和IL-17的浓度(12.21±1.79pg/ml,109.07±3.02pg/ml,P0.05);使用常规化疗药物治疗后TB病人血清IL-22和IL-17的浓度分别为15.12±1.08pg/ml和115.63±2.98pg/ml,均显著低于治疗前IL-22和IL-17的浓度水平(26.70±10.63pg/ml,128.02±6.96pg/ml,P0.05);痰涂片TB菌检测阳性患者IL-22和IL-17的浓度分别为30.68±9.13pg/ml和131.67±4.96pg/ml,均显著高于痰涂片TB菌检测阴性患者的水平(19.89±2.54pg/ml,125.15±4.58pg/ml,P0.05)。Pearson相关性分析显示,TB患者外周血清IL-22的浓度和IL-17的浓度呈显著正相关(P0.05),且TB患者血清中IL-22和IL-17的浓度与ESR、CRP的水平均为正相关(P0.05),与WBC的水平无明显相关性(P0.05);痰涂片TB菌阳性患者血清IL-22的浓度和IL-17的浓度呈显著正相关(P0.05),且TB菌阳性患者血清IL-22和IL-17的浓度分别与WBC、ESR、CRP的水平均为正相关(P0.05)。结论 IL-22和IL-17在机体的抗结核免疫过程中发挥重要作用。

Expression and clinical significance of serum IL-22 and IL-17 in patients with pulmonary tuberculosis
Abstract:Objective To investigate the expression levels and clinical significance of serum IL-22 and IL-17 in pulmonary tuberculosis patients. Methods The serum levels of IL-22 and IL-17 were quantified by using enzyme-linked immunosorbent as-says (ELISA) in 59 pulmonary tuberculosis patients (TB) and 44 healthy donors (HD). At the same time WBC ,ESR and CRP were also detected. Pearson correlation analysis was used to analyze the correlation between cytokines and three inflammatory prefer-ences. Results The concentrations of serum IL-22 and IL-17 in TB group were respectively 26.70 ±10.63pg/ml and 128.02 ±6. 96pg/ml,which were both significantly higher than in HD group (12.21±1.79pg/ml,109.07±3.02pg/ml,both P<0.05);the concentra-tions of serum IL-22 and IL-17 in TB patients after routine chemotherapy treatment were respectively 15.12 ±1.08pg/ml and 115. 63±2.98pg/ml,which were both significantly lower than that before treatment (26.70±10.63pg/ml,128.02±6.96pg/ml,both P<0.05);the concentrations of serum IL-22 and IL-17 in sputum smear positive TB group were respectively 30.68±9.13pg/ml and 131.67± 4.96pg/ml,which were both significantly higher than that in sputum smear negative TB group(19.89±2.54pg/ml,125.15±4.58pg/ml, both P<0.05). There was a significant positive correlation of serum IL-22 with IL-17in TB group (P<0.05),and it was the same be-tween cytokines and ESR, CRP(both P<0.05),but there was no significant correlation between cytokines and WBC in TB group (P>0.05);there was also significantly positive correlation between serum IL-22 and IL-17 in sputum smear positive TB group ,and it was the same result between cytokines and WBC,SR,CRP in sputum smear positive TB group (both P<0.05). Conclusion These findings indicated that IL-22 and IL-17 may play an important role in the immune response against Mycobacterium tuberculosis.
