摘    要:目的了解本地区妇女宫颈脱落细胞HPV的感染情况和基因型分布特点,观察其在不同年份、不同年龄段、不同亚型的分布特点,为本地区HPV的筛查和防治提供依据。方法以2011年1月至2015年8月来我院妇产科门诊就诊的妇女为研究对象,按照年龄分为6个组即(T120岁,T2:21~30岁,T3:31~40岁,T4:41~50岁,T5:51~60岁,T6:60岁)采集宫颈脱落细胞,使用深圳亚能公司生产的试剂进行HPV基因型的检测,一共可以检测23种常见的基因亚型。将得到的测试数据按照年份排序,统计HPV总感染率,总体基因亚型分布特点,再比较不同年份,不同年龄,不同亚型的感染情况。结果 1总共检测20400例标本,HPV阳性的标本数为4466例,总感染率为21.89%。2因为存在多重感染(两种或两种以上的基因亚型感染)的情况,4466例阳性的标本中23种HPV常见基因型共出现7024次,其中高危型占72.72%(5108/7024),以HPV52、16、58三种亚型为主分别占14.24%(1000/7024)、10.75%(755/7024)和7.30%(513/7024),低危型占27.28%(1916/7024),以HPV43、6、11三种亚型为主,分别占9.38%(659/7024)、8.93%(627/7024)、5.11%(359/7024)。3HPV感染的高峰期出现在T1、T5和T6组,分别为41.34%、31.65%、32.93%。随着年龄的增加低危型感染率逐渐降低,高危型感染率逐渐升高,各年龄组高危型感染率均大于低危型感染率。4患者HPV单一种基因型感染率为66.41%(2966/4466),多重感染率为33.59%(1500/4466),多重感染主要出现在15~40岁之间,其中最多出现了9重感染。52011-2015年各年份HPV总感染率的差异没有统计学意义,但是HPV6型感染百分比逐年降低,HPV51、52型感染百分比逐年升高。结论本地区HPV感染率高达到21.89%,以高危型感染为主,HPV基因型在不同年份和不同年龄组有差异,因此加强HPV的筛查很有必要。

Analysis of the infection and genotype distribution of 20400 HPV patients in Dongguan
Abstract:Objective To investigate the infection and genotype distribution of human papillomavirus (HPV) in Dongguan base on the year,age and subtype,so as to provide evidence for HPV screening and prevention in Dongguan. Methods The patients who were from outpatient service of department of gynaecology of our hospital from January 2011 to August 2015 were collected to gather cervical cytological speciments,23 common genotypes of HPV were detected by analysis kits (YANENGBIO,Shenzhen). Six groups (T1-T6) were set up based on the age. A retrospective analysis was carried out to evaluate the overall prevalence of HPV in-fection. The prevalence of HPV infecton in different ages and subtypes were compared. Results ①Atotal of 20400 specimen were collected,and the overall HPV infection rate were 21.89%(4466/20400). ②The more common high risk genotype and low risk genotypes were HPV52,HPV16,HPV58 and HPV43,HPV16,HPV11 respectively. Conclusion The screening of HPV genotypes for healthy person is meaningful in early prevention of cervical cancer and epidemiological studies of HPV ,and we should pay more attention in the prevention of HPV for young and elderly people.
