急性脑梗死介入治疗的疗效观察

摘    要:目的观察急性脑梗死患者行动脉内溶栓治疗的临床疗效。方法41例急性脑梗死患者采用超选择脑动脉内溶栓方法,起病距溶栓时间3~24h,尿激酶以1.0万U/min持续泵入,总量为120万U,辅助其它常规治疗,治疗14d后评价其临床疗效。结果溶栓有效率为87.8%,显效率68.3%,血管再通率为80.48%,治疗后神经功能缺损评分较前显著降低(P〈0.01)。结论动脉内溶栓治疗急性缺血性脑梗死是一种安全有效的方法,可使大部分患者闭塞动脉再通,提高临床治疗效果。

Abstract:Objective To present a retrospective analysis of the clinical outcome in patients treated by local intra-arterial thrombolysis (LITT) with urokinase for acute ischemic stroke of the anterior or posterior arterial circulation. Methods 41patients with acute cerebral infarction were treated by superselective infra-arterial thrombolysis in 3-24hours after onset of the symptoms of cerebral infarction. Urokinase (UK) was infused into the infracted cerebral artery at the rate of 10 000 unit/min. The total dosage of UK was 1 200 000 units. The patients were given routine treatment. The curative effect were evaluated 14 days after the treatment. Results The rate of effective, excellence, recanalization were 87.8% and 68,3% and 80.48% respectively.The neurological deficit scale score was significantly lower than before (P〈0.01). Conclusion Intra-arterial thrombolysis is a safe and effective therapy for acute ischemic cerebral infarction.
