直径小于3cm肺结核球和周围型肺癌的CT诊断

目的:探讨直径<3cm的结核球和周围型肺癌的CT特征及其鉴别诊断。材料与方法:分析直径<3cm的31例结核球和56例周围型肺癌的CT影像特征。结果:31例结核球中,21例边缘光滑、出现钙化17例、小空洞11例、胸膜增厚14例;56例周围型肺癌中,边缘毛糙、有分叶者35例、胸膜凹陷征17例、纵隔或肺门淋巴结肿大13例。增强扫描16例结核球无强化、4例有薄层环状强化;而38例周围型肺癌增强后均有强化,其中不均匀强化32例、均匀强化6例。结论:CT平扫及增强扫描清晰显示直径小于3cm肺结核球和周围型肺癌的病变特征及血供情况,有助于诊断及鉴别诊断。

CT diagnosis of tuberculomas and peripheral carcinomas of lung with diameters less than 3cm

Objective:To find out CT characteristics of tuberculomas and pulmonary carcinomas with diameters less than3cm,and to evaluate its value in differentiating the two diseases.Material s and Methods:CT manifestations of 87patients including31tuberculomas and56pulmonary carcinomas with diameters less than3cm were analyzed.Result:Twenty-one cas-es of 31tuberculomas had smooth edge,17calcifications,11showed small cavity,and14pleural-thicking.Thirty-five cases of 56pulmonary carcinomas showed shaggy border and deep lobulation,17pleural indentation,and13had big mediastinal and hilar lymph nodes.After injection of iodinated contrast material16of 20tuberculomas showed nonenhancement ,and4ring enhancement,32of 38pulmonary carcinomas showed inhomogeneous enhancement,and6homogeneous enhancement.Con-clu sion:Thin-slice CT scanning and enhancement were a valuable method for differentiating tuberculomas from pulmonary car-cinomas.
