外周动静脉双管同步换血在新生儿高胆红素血症中的临床应用

摘    要:目的探讨治疗新生儿高胆红素血症的换血方法。方法采用外周动静脉双管同步换血(输入和输出均由容量泵控制同步进行)治疗新生儿高胆红素血症。结果换血后15例新生儿高胆红素血症患儿,总胆红素由(470.45±32.43)mmol/L降至(250.37±16.50)mmol/L,间接胆红素由(378.78±20.54)mmol/L降至(182.74±19.18)mmol/L,治疗前后比较,差异显著(均P<0.01);15例高胆红素血症患儿均病情好转。结论外围动静脉双药同步换血是清出患儿体内毒素的有效方法,且操作方便,安全性高。我们认为,做好术前准备,术中密切观察生命体征,保证输入与输出血量的动态平衡等是该疗法的关键,可在临床推广应用。

Abstract:Objective To discuss the treatment method of exchange transfusion in neonates with hyperbilirubi- hernia. Methods 15 neonatal with hyperbilirubinemia was treated with double input and output by the capacity of pump control synchronization. Results The total hilirubin decrease form (470.45 ±32.43) mmol / L to (250.37± 16.50) mmol / L, indirect bilirubin from (378. 78±20.54) mmol / L To (182.74±19.18) mmol / L after treatment. 15 cases were in better condition. Conclusion It is effective to treat neonates with hyperbilirubinemia with double input and out- put by the capacity of pump control synchronization,
