注射用阿魏酸钠治疗糖尿病肾病84例临床观察

摘    要:目的观察注射用阿魏酸钠对不同时期糖尿病肾病患者肾功能及尿蛋白的影响。方法糖尿病肾病患者84例随机分为试验组和对照组各42例。两纽患者均根据尿蛋白和肾功能的不同情况再分别分为临床期肾功能正常组和临床期肾功能不全组两个亚组。对照组给予保肾排毒中成药及控制血压、血糖等治疗方法,试验组给予注射用阿魏酸钠,每日1次,连续4周,余治疗同对照组。观察治疗前及治疗4周后肾功能改善情况。结果与治疗前相比,治疗组中肾功能正常亚组血清肌酐、尿素氮下降(P〈O.05),尿蛋白明显下降(P〈O.01);肾功能不全亚组血清肌酐、尿素氮、尿蛋白均下降(P〈O.05);对照组中两亚组上述指标亦较治疗前下降,但与治疗前相比无统计学意义(P〉0.05)。与相应对照组相比,治疗前各亚组上述指标无显著差异(P〉0.05);治疗后肾功能正常亚组血清肌酐、尿素氮下降(P〈0.05),尿蛋白明显下降(P〈O.01);肾功能不全亚组血清肌酐、尿素氮、尿蛋白均下降(p〈0.05)。结论注射用阿魏酸钠可进一步改善不同时期糖尿病肾病患者肾功能及尿蛋白情况。

Abstract:Objective To study the effects of sodium ferulate on proteinuria and renal function in patients with dia- betic nephropathy. Methods 84 patients with diabetic nephropathy were divided into two groups at random. According to the level of proteinuria and renal function, the patients of each group were divided into 2 subgroups (clinical stage with normal and abnormal renal function). The controls were treated with Chinese medicine to eliminate the toxicant and con- trol blood pressure and blood sugar levels. The patients in the study group were treated with intravenous infusion of sodi- um ferulate every day for 4 weeks. Other treatment was the same to the controls. Results After treatment, the serum creatinine and urea nitrogen in the clinical normal renal function subset of the study group were significantly reduced com- pared with those of before (P〈0. 05), especially the quantity of urine protein during 24 hours (P〈0. 01). While those indices in renal inadequacy subset in the study group also decreased, but no significance were found before and after the treatment between the subsets (P〉0.05). However, when compared with each controls, those indexes in the clinic nor- mal renal function had no significance before treatment (P〉0. 05). While after treatment the quantity of urine protein during 24 hours, the serum creatinine and urea nitrogen in the renal inadequacy subset of the study group were signifi- cantly reduced(P〈0.05). Conclusion Sodium ferulate may decrease urine protein and improve the renal function during distinct stages of diabetic nephropathy.
