Medial knee arthritis is a common indication for a knee endoprosthesis. Various factors have an influence on the selection of the treatment procedure. Bicompartmental prostheses in young active patients are not recommended due to the expected survival time of the implant. Alternatives were evaluated which can reduce pain and at the same time preserve all options for knee prosthetics. The UniSpacer is a metal spacer for the medial compartment. The spacer is inserted unfixed into the medial tibial plateau with the aim of a reconstruction of the physiological biomechanics of the knee. Indications for this implant have to be chosen carefully. As expected pain relief is limited compared to endoprostheses; clinical studies have shown relatively high revision rates. Revision surgery to any other type of implant can be performed easily.
