慢性肾功能衰竭心脏损害的临床分析

目的:探讨慢性肾功能衰竭(慢性肾衰)的心脏损害情况.方法:对120例慢性肾功能衰竭患者进行回顾性分析,并以同期住院肾功能正常的肾脏病为对照组.结果:慢性肾衰与对照组比较,左心房内径(LAD)、左心室舒张末期内径(LVEDD)增大(P<0.01),室间隔厚度(IVS)、左室后壁厚度(LVPW)增加(P<0.01),左室舒张末期容量(EDV)及收缩末期容量(ESV)增加(P<0.01),射血分数(EF)降低(P<0.05).41.6%有左室壁运动降低,19.1%有心包积液,31.5%出现心力衰竭.肾功能损害程度、高血压及贫血与心脏损害有密切关系.结论:慢性肾衰存在不同程度的心脏损害,特别是心脏结构和功能异常,在尚未进入终末期肾衰前即已发生,心脏超声检查是发现心脏结构和功能损害及心包积液的敏感方法,应作为慢性肾衰的常规检查.

Clinical Investigation on Heart Damage Due to Chronic Renal Function Failure

Objective: To study the heart damage condition following chronic renal failure(CRF).Methods:Comparing to the cases with normal renal function, analysis on 120 cases of CRF. Results: Comparing to those in control group, the levels of LAD, LVEDD, LVS, LVPW and ESV in CRF group were higher(P<0.01), while the level of EF was lower(p<0.05). There were 41.6% lower activity in LV, 19.1% liquid around heart membrane and 31.5% occurred heart function failure. There were closely connected among chronic renal failure damage, high blood pressure, deficiency blood and heart damage. Conclusion:There are different levels of heart damage in case of CRF. Especially, there are some abnormal structure and function of heart before last period of CRF. Checking with heart B-us is a sensitive way to find heart structure and function damage, which should be normally used in checking CRF.
