玻璃体视网膜手术硅油充填术后高眼压的临床治疗

目的:探讨玻璃体视网膜术手术硅油充填术后高眼压的治疗方法。方法:对玻璃体视网膜手术硅油充填术后,眼压大于等于24mmHg的患者96眼,进行药物或手术治疗。结果:85眼(占高眼压总数的88.54%)经过单一或联合多种药物治疗眼压控制。2眼经睫状体平坦部放出1～2滴硅油,眼压控制。硅油进入前房者:有晶体眼中2眼经前房放油眼压控制,其中1眼硅油再次进入前房,硅油填充亦超过1个月,经保守治疗无效,检查眼底视网膜复位后,同时行前房和玻璃体腔硅油取出术;无晶体眼中3眼经俯卧位加点降眼压药物,眼压控制,2眼虹膜周切孔膜闭者经过激光6点位打孔或重复打孔处理眼压控制,1眼作6点位周切孔重建,1眼行虹膜周切联合前房成形术,眼压控制。96眼眼压控制后,视力均有不同程度提高。结论:高眼压是视网膜脱离患者玻璃体视网膜手术后比较常见的并发症,硅油充填增加了术后高眼压的发生率,药物、手术等综合治疗可有效控制眼压。

Clinical therapy of ocular hypertension after operation on retina and vetreous of silicon oil plumbage

Objective:To explore Clinical therapy of ocular hypertension after Operation on retina and vetreous of silicon oil plumbage.Methods:A total of 96 eyes intraocular tension exceed-and-equal 24mmHg after retina and vetreous of silicon oil plumbage operation were treated by the medicine or surgery.Results:intraocular tension of 86 eyes(89.6% of the number of intraocular hypertension)was been controlled after single or affiliation several drug treatment.Two weys,s intraocular tension wae controlled after releasing 1~2 drops of silicon oil.silicon oil fit-into anterior chamber:two crystal eyes,intraocular tension has been controlled by releasing silicon oil,silicon oil fit-into one eye again,silicon oil filling exceed one month,useless of expectant treatment,after checking the reattachment of retina,doing the silicon oil dislodge from anterior chamber and vitreous cavity;there aphakia eyes by prone position and droping hypotensive drug:two iris ambitus excision aperture membrane occlusor eyes by lasering at 6 clock or perforating again.,iris ambitus excision at 6 clock by lasing,one by iris ambitus excision affiliation anterior chamber plasty.acuity of vision has been elevated different degree after intraocular tension controlled.Conclusion:intraocular hypertension is a frequent complication after Operation on retina and vitreous of detached retinal patients,the filling of silicon oil accrescenced the incidence rate of ocular hypertension.The ocular hypertension can be controlled by medicine、surgery and so on.
