幽门弯曲菌与胃十二指肠炎症

观察了51例非溃疡病消化不良或消化性溃疡病人的胃体、胃窦和十二指肠球部胃镜活检组织中幽门弯曲菌(Campylobacter pyloridis,CP)与胃十二指肠炎症的关系。细菌培养、组织切片和电镜观察表明,CP为革兰氏染色阴性、螺旋或短弧形杆状,具鞭毛,培养微需氧并含高活性尿素酶。胃体、胃窦和球部活检CP阳性率分别为55.9％、75.4％和21.4％,差异显著(P<0.01),胃窦部阳性率最高可能与胃窦炎高发有关。正常胃粘膜中均无CP.CP与胃慢性炎症,尤其与活动性炎症密切相关(P<0.01)。球部CP阳性率与球部炎症和溃疡无明显相关(P>0.05)。14例经铋剂治疗炎症好转者CP减少或消失。结果进一步证实CP与慢性胃炎有关。

Campylobacter pyloridis and Gastroduodenal Inflammation

65 serial gastroduodenal biopsies were taken randomly from 51 patients with non-ulcerative dyspepsia or peptic ulcer. Each biopsy consisted of 3 specimens from gastric body, antrum and duodenal cap. In culture and tissue section as wellas under electron microscopy, Campylobacter pyloridis (CP) was demonstrated to be gram-negative, spiral or curved rod-like, flagellate and microaerophilic with strong urase activity. 76.9% of 65 serial biopsies were CP positive. The incidence of CP in the mucosa of gastric body, antrum and duodenal cap were 55.9%, 75.4% and 21.4% respectively, the differences between them being statistically significant (P<0.01). The presence of CP in gastric mucosa correlated well with chronic gastritis, especially with active gastritis evidenced by infiltration of neutrophils (P<0.01). CP was not identified in all 19 gastric specimens showing normal histology. No obvious association was found between presence of CP and active duodenitis or duodenal ulcers. In 14 patients with confirmed CP infection, remission of active gastritis after treatment seemed to be related to the elimination of CP, thus supporting the view that CP may play an important role in the etiology of chronic gastritis.
