多层螺旋CT和彩色超声对乳腺癌的诊断价值及肿块大小分析比较

目的:探讨多层螺旋CT(MSCT)和彩色超声(US)对乳腺癌的诊断价值,为临床早期优化诊断提供参考。方法:收集西南医科大学附属医院2017年1-12月期间90例主诉乳腺肿块患者,入院后均接受常规US和MSCT检查。以手术病理检查为诊断金标准,观察常规US和MSCT的影像特征,并计算二者诊断乳腺癌的准确度、灵敏度和特异度。结果:手术病理证实良性肿块42.22%,恶性肿块57.78%。参照金标准,MSCT诊断的准确度92.22%、灵敏度90.38%分别高于US检查81.11%、75.00%,差异有统计学意义(P <0.05),二者特异度分别为94.74%、89.47%,相较无显著差异(P>0.05);根据乳腺肿块大小统计,MSCT、US对≥1.1cm的乳腺肿块检出均较好,诊断准确率分别为90.54%、95.95%,相较无显著差异(P>0.05),但MSCT对≤1.0cm的肿块诊断准确率75.00%高于US检查的37.50%,差异有统计学意义(P<0.05)。结论:MSCT对乳腺癌的诊断效果优于US,对微小乳腺肿块也能较好检出诊断,可作为常规US检查的重要补充手段。

The Value of Multi-slice Spiral CT and Color Ultrasound in the Diagnosis of Breast Cancer and Analysis of Tumor Size

Objective: To evaluate the value of multi-slice spiral CT(MSCT) and color Doppler ultrasound(US) in the diagnosis of breast cancer, and to provide a reference for clinical early diagnosis. Methods: A total of 90 patients with breast mass complained in our hospital from January 2017 to December 2017 were examined by US and MSCT after admission. The diagnostic gold standard is surgical pathological examination. The imaging features of US and MSCT were observed and the accuracy, sensitivity and specificity of US and MSCT were calculated. Results: 42.22% benign masses and 57.78% malignant masses were confirmed by operation and pathology. According to the gold standard, the diagnostic accuracy and sensitivity of MSCT were 92.22% and 90.38% respectively, higher than those of US 81.11% and 75.00% respectively. The difference was statistically significant(P<0.05). The specificity of MSCT and US were 94.74% and 89.47% respectively, with no significant difference(P>0.05). According to the statistics of breast mass size, MSCT and US are better for breast mass ≥1.1 cm. The diagnostic accuracy was 90.54% and 95.95% respectively. There was no significant difference between them(P>0.05), but the accuracy of MSCT in the diagnosis of mass ≤1.0 cm was 75.00% higher than that of US 37.50%(P<0.05). Conclusion: MSCT is superior to US in the diagnosis of breast cancer. MSCT can also detect and diagnose small breast masses. MSCT can be used as an important supplementary means for routine US examination.
