门冬胰岛素与甘精胰岛素联合治疗妊娠期糖尿病的疗效观察

目的探究门冬胰岛素联合甘精胰岛素对妊娠期糖尿病(GDM)患者的临床治疗效果。方法48例妊娠期糖尿病患者,按随机数字表法分为试验组和对照组,各24例。试验组患者接受门冬胰岛素联合甘精胰岛素治疗,对照组患者接受甘精胰岛素单独治疗。比较两组患者并发症发生情况、治疗前后血糖水平、血糖达标率及低血糖事件发生情况。结果试验组患者的并发症发生率8.33%低于对照组的33.33%,差异具有统计学意义(P<0.05)。治疗后,试验组患者的空腹血糖(5.32±0.78)mmol/L、餐后2 h血糖(11.64±1.05)mmol/L及糖化血红蛋白(5.25±0.48)%均低于对照组的(7.22±1.04)mmol/L、(13.34±1.22)mmol/L、(6.84±0.66)%,差异均具有统计学意义(P<0.05)。试验组血糖达标率75.00%(18/24)高于对照组的45.83%(11/24),差异具有统计学意义(χ^2=4.2686,P<0.05);两组患者低血糖事件发生率比较差异无统计学意义(χ^2=0.2233,P>0.05)。结论妊娠期糖尿病患者接受门冬胰岛素联合甘精胰岛素治疗的效果显著,可有效控制患者血糖水平,降低并发症发生几率,临床应用价值突出。

Observation on efficacy of combination of insulin aspart and insulin glargine on gestational diabetes mellitus

Objective To investigate the clinical therapeutic effect of insulin aspart combined with insulin glargine on patients with gestational diabetes mellitus(GDM).Methods A total of 48 patients with gestational diabetes mellitus were divided into experimental group and control group by random number table method,with 24 cases in each group.The experimental grouy was treated by insulin aspart combined with insulin glargine,and the control group was treated by insulin glargine.The occurrence of complications,blood glucose levels before and after treatment,blood glucose compliance rate,and occurrence of hypoglycemic events were compared between the two groups.Results The incidence of complications of the experimental group 8.33%was lower than that of the control group 33.33%,and the difference was statistically significant(P<0.05).After treatment,the fasting blood glucose(5.32±0.78)mmol/L,2 h postprandial blood glucose(11.64±1.05)mmol/L and glycosylated hemoglobin(5.25±0.48)%of the experimental group were lower than those of the control group(7.22±1.04)mmol/L,(13.34±1.22)mmol/L and(6.84±0.66)%,and the difference was statistically significant(P<0.05).The blood glucose compliance rate of the experimental group 75.00%(18/24)was higher than that of the control group 45.83%(11/24),and the difference was statistically significant(χ^2=4.2686,P<0.05).There was no statistically significant difference in incidence of hypoglycemia events between the two groups(χ^2=0.2233,P>0.05).Conclusion Combination of insulin aspart and insulin glargine shows remarkable effect on patients with gestational diabetes mellitus,which can effectively control the blood glucose level and lower incidence of complications.The clinical application value is outstanding.
