腹腔镜手术与开腹手术治疗早期子宫颈癌和内膜癌的对比分析

目的　探讨腹腔镜手术治疗早期子宫颈癌和内膜癌的价值。方法　回顾性分析11 例经腹腔镜手术和12例经开腹手术治疗的临床Ⅰ期子宫恶性肿瘤患者的临床资料,比较两组的手术时间、术中出血量、并发症、术后恢复情况及复发率等。结果　两组间手术成功率、手术时间、术中出血量、并发症、淋巴结切除数及术后恢复情况的差异均无显著性(P>0.05)。腹腔镜组术后住院时间明显减少,平均为(7.3±2.5) d,而开腹组为(9.7±2.8) d(P<0.05)。经随访,腹腔镜组1例宫颈癌患者于术后6 个月发现有盆壁淋巴结转移。结论　从短期疗效看,腹腔镜手术可以完成广泛全子宫切除和淋巴结清扫,可作为子宫恶性肿瘤手术治疗的方法之一。

Comparative analysis between laparoscopic operation and laparotomy in treating early stage of cervical and endometrial carcinomas

Objective To investigate the clinical value of laparoscopic operation in treating early stage of cervical and endometrial carcinomas. Methods A retrospective analysis was carried out to compare the clinical parameters of laparoscopic operation and laparotomy, such as operative time, blood loss, complication, postope-rative recovery and recurrence.Results There were no significant differences between the two groups concerning about the successful rate, operative time, blood loss, complications, number of lymph nodes excision and posto-perative re covery.The durations of hospitalization were 7.3 days in laparoscopic grou p and 9.7 days in laparotomic group respectively (P< 0.05). Only one p atient with cervical carcinoma undergone laparoscopic operation, showed lymph nodes metastasis in pelvic wall after six months follow up. Conclusion Radical laparoscopic hysterectomy and pelvic lymphadenectomy could be used as an alternative method in treating early stage of cervical and endometrial carcinomas.
