无皮肤烧伤的严重烟雾吸入性损伤的救治

目的:总结无皮肤烧伤的严重烟雾吸入性损伤的救治经验;方法:共收集4例(2批)病例.第一批入院诊断为重度吸入性损伤,吸纯氧后气管切开;第二批人院诊断为中度吸入性损伤,人院4小时后呼吸困难加重被迫气管切开.两批患者均配合药物治疗.结果:第一批病人伤后14天拔除气管套管:28天治愈;第二批伤后9天拔除套管,23天治愈.结论:即使尤皮肤烧伤,只要有密闭环境内烟雾吸入史,也要首先警惕吸入性损伤.一旦诊断为中重度吸入性损伤,尽早气管切开,采取综合治疗措施,使此类病人能得到及时、有效的救治.

Experience of Treating Severe Smoke Inhalation Injury Cases of No Skin Burn Injury

Objective： To summarize the experience of treating severe smoke inhalation injury cases of no skin burn injury. Method： Total 4 cases （2 batches） of patients with severe smoke inhalation injury were collected. The first batch of the patients were admitted with diagnosis of severe inhalation injury and received tracheotomy immediately after pure oxygen inhalation. In the second batch, the patients were admitted with diagnosis of moderate and severe inhalation injury and were forced to do traeheotomy 4hrs later of admission because of aggravation of breathing difficult. Results： The tracheal tube of the patients in the first batch were removed on 14^th day after injury and the patients were cured on 28^th day. The tracheal tube of the patients in the second batch were removed on 9^th day after injury and they were cured on 23^rd day. Conclusion： Even no skin injury, it should keep eyes on the inhalation injury firstly so long as the patients has the history of smoke inhalation in the confined environment. Once moderate and severe inhalation injury was diagnosed, tracheotomy should be conducted as soon as possible. Meanwhile, system treatment measures must be taken so that the patients can obtain the treatment timely and effectively.
