老年慢性心力衰竭患者血浆内皮素与血清前胶原肽相关性研究

目的研究老年慢性心力衰竭（CHF）患者血浆内皮素-1、血清Ⅰ型前胶原羧基端肽（PⅠCP）、Ⅲ型前胶原氨基端肽（PⅢNP）含量的变化及其临床意义。方法采用放射免疫分析法分别测定心衰患者血浆内皮素-1（ET-1）、PⅠCP和PⅢNP含量,同时用心脏多普勒超声检查心脏左心房内径、LVEF%,研究血浆ET-1、PⅠCP、PⅢNP的含量变化及相关性。结果心衰患者ET-1、PⅠCP、PⅢNP含量较对照组明显升高（P〈0.05或P〈0.01）;且ET-1与血清PⅠCP、PⅢNP含量均呈显著正相关（r=0.574,P〈0.01;r=0.596,P〈0.01）,血清PⅠCP与PⅢNP含量也呈正相关（r=0.688,P〈0.01）。结论老年心衰患者ET-1、PⅠCP、PⅢNP含量增加,内皮素与心肌纤维化密切相关。

Study of correlation between plasma endothelin and serum procollagen peptide in elderly patients with chronic heart failure

Objective To investigate the changes of plasma endothelin 1（ET-1）,serum procollagen typeⅠC-terminal peptide（PⅠCP）and serum procollagen typeⅢ N-terminal peptide（PⅢNP）levels and their clinical significance in elderly patients with chronic heart failure（CHF）.Methods The contents of plasma ET-1,serum PⅠCP and PⅢNP from 66 elderly patients with CHF were determined by radioimmunoassay.Meanwhile,left atrial diameter and left ventricular ejection fraction were measured by Doppler echocardiography.The correlations among above three indexes were analyzed.Results ET-1,serum PⅠCP and PⅢNP contents in CHF group were significantly higher than those in control group（P0.05 or P0.01）.Moreover,ET-1 level was positively correlated with PⅠCP（r=0.574,P0.01）and PⅢNP（r=0.596,P0.01）.PⅠCP and PⅢNP contents also demonstrated a positive correlation（r = 0.688,P0.01）.Conclusion The contents of plasma ET-1,serum PⅠCP and PⅢNP all increase in elderly patients with CHF.ET is closely correlated with myocardial fibrosis.
