急性上消化道出血后贫血患者EPO与贫血程度的相关性研究

目的探讨急性上消化道出血后贫血患者的促红细胞生成素（EPO）随时间变化趋势及其与贫血程度之间的关系。方法本研究纳入急性上消化道出血后贫血患者68例,以30例正常人作为对照。采集上消化道出血后贫血患者出血后第1、4、7 d的空腹外周静脉血各5 ml,而正常对照组仅采集1次;以双抗体夹心酶联免疫吸附方应法（ELESA）测定血清EPO浓度,比较上消化道出血后贫血患者各时间点EPO水平与对照组之间的差异。此外,将纳入患者按照贫血程度分为轻中重3个亚组,对EPO水平进行比较分析。结果急性上消化道出血组出血后EPO水平呈现先升高后降低的趋势,第4 d的EPO水平显著高于正常组（P〈0.05）,3个时间点之间的比较无统计学差异。重度贫血组EPO水平高于正常组、轻度贫血组和中度贫血组（P〈0.05）。结论在急性上消化道出血后贫血患者EPO水平具有随时间先升高后降低的趋势,重度贫血患者的EPO水平最高。

Correlation between erythropoietin level and anemic degree in anemic patients following hemorrhage of upper digestive tract

Objective To investigate the time-variant tendency of erythropoietin（EPO）and its relationship with anemic degree.Methods 68 anemic patients following upper digestive tract were included in this study,and 30 healthy individuals were selected as control group.5 ml of peripheral venous blood samples from above patients was respectively collected 1,4 and 7 d after hemorrhage,and that of healthy controls was only collected once.Serum EPO level was measured using ELISA,and the findings at different time points were compared with control group.Additionally,anemic patients were divided into mild,moderate and severe subgroups according to the anemic degree,and EPO levels among them were compared.Results EPO level in anemic patients at three time points raised firstly and then dropped.It increased significantly on day 4（P0.05）,and there were no significant differences among the three time points.Severe anemic group had higher EPO level than that in mild anemic group,moderate anemic group and control group（P0.05）.Conclusion EPO level in anemic patients following upper digestive tract appears to have a first increased and later decreased tendency.It is the highest in those with severe anemia.
