变应原皮肤点刺和CAP过筛检测在儿童哮喘中的应用价值

目的 探讨变应原皮肤点刺试验和CAP过筛检测在儿童哮喘中的应用价值。方法 采用上海医科大学螨类研究室提供的变应原皮试液行皮肤点刺试验。用瑞典Pharmacia公司研制的CAP变应原检测系统进行Phadiatop和fx5E过筛检测。将两种检测结果分组进行比较。结果 142例儿童哮喘中。皮肤点刺阳性102例（71．8％），CAP变应原检测阳性108例（76．1％）。吸入性变应原阳性率分别为71．1％和75．4％。阳性率随年龄增加而增加。食物性变应原阳性率分别为19．7％和28．2％。阳性率与年龄之间无明显规律性。吸入性变应原阳性率和食物性变应原阳性率，两种检测结果的总符合率分别为73．2％和64．8％。结论 哮喘儿童开展变应原检测，应合理利用两种检测方法的互补性，对提高变应原检测的阳性率和降低医疗成本是行之有效的方法。

Value of allergen skin prick test and CAP sieving detection in children with asthma

Objective] To approach the value of allergen skin prick test and CAP sieving detection in children with asthma. Methods] The allergic liquid for test is provided by dermatophagoides farina laboratory of Shanghai medical university. The sieving detection about Phadiatop and fx5E is provided by the CAP anaphylactogen detection system. We compare the results between different groups divided by test results, respectively. Results] In all the 142 cases of childhood asthma, 102 (71.8%) are positive in skin prick test and 108 (76.1%) are positive in CAP detection. Inhalant allergens positive rate are 71.1% in skin prick test and 75.4% in CAP detection, the positive rate of inhalant allergens increase with age. Food allergens positive rate are 19.7% in skin prick test and 28.2% in CAP detection, the positive rate of food allergens are not related to age. The total coincidence of positive rate of inhalant allergens and food allergens in two kinds of detection are 73.2% and 64.8%, respectively. Conclusion] It is valid method to improve the positive rate of allergy detection and to decrease the medical cost in children with asthma. We should reasonably use the complementarily of two kinds of the detection system.
