唇腭裂患者围手术期的饮食干预

目的 探讨唇腭裂患儿围手术期的正确饮食程序。方法 对2009年1月～8月我科100例先天性唇腭裂患儿随机分为观察组和对照组,每组50例。 观察组从患儿入院、术前、术后、出院均按照详细的饮食干预计划执行。对照组只在术前、术后进行饮食干预 。结果 两组患儿手术顺利,观察组患儿术后饥饿哭闹的发生率为50%、伤口渗血的发生率6%、伤口感染的发生率4%、伤口裂开的发生率0。对照组患儿术后饥饿哭闹的发生率为88%、伤口渗血的发生率12%、伤口感染的发生率10%、伤口裂开的发生率4%。观察组较对照组,适应术后进食要求的时间,切口的感染、裂开,术后感染发生率均明显降低,差异具有统计学意义（P〈0.05）。结论 早期进行饮食干预,制定规范化饮食流程,明显减少了护理并发症,缩短了患儿住院天数,降低了住院费用。

The dietary intervention for child patients with chilopalatognathus during perioperation

Objective To study the proper procedure of dietary intervention for child patients with chilopalatognathus during perioperation.Methods 100 congenital child patients with chilopalatognathus during January 2009 to August 2009 were randomly divided into observation group and control group with 50 cases for each.Patients in control group have accepted diet intervention during operation whereas patents in observation group were given diet intervention before and after operation besides during operation.Results The operations of patients in both groups were done successfully. The incidence of hunger cry of patients after operation in observation group was 50%, bleeding incidence 6%, wound infection incidence 4%, dehiscence incidence 0 ; whereas hunger cry incidence in control group was 88%, bleeding incidence 12%, wound infection incidence 10%, dehiscence incidence 4%. Compared with that in control group, patients in oberservation group adapted more quickly to post-operative diet schedules, the incidences of infection, dehiscence and post-operative infection reduced statistically significantly. （P〈0.05）. Conclusion Early diet intervention and standardized diet administration could reduce nursing complications , hospital cost and shorten hospital days.
