腓肠神经营养血管皮瓣的临床应用

目的 探讨腓肠神经营养血管皮瓣顺行转位修复小腿上段软组织缺损。逆行转住修复小腿中下段及足部软组织缺损的临床应用效果、探讨皮瓣转位面积及血管蒂部的处理方案。方法 在小腿后侧以腓肠神经体表投影为轴心线，设计及切取腓肠神经营养血管筋膜皮瓣，顺行转位修复小腿上段软组织缺损6例，逆行转位修复小腿中下段及足部软组织缺损51例。结果 54例皮瓣完全成活,3例皮瓣远端部分坏死，皮下组织成活，经植皮愈合，经随访皮瓣外观、质地、功能良好。结论 腓肠神经营养血管皮瓣血供可靠，操作简单，可以修复胫前区、踝部及足部跖骨中段近侧软组织缺损。

Clinic application of sural neurovascular flap

Objective To explore the clinical application effect of sural neurovascular flap to be transfered for repairing the tissue defect around the leg and the foot,the size of flap and how to deal with the length of vessel pedicle. Methods According to the anatomy of sural nerve,to plan the sural neurovascular fascial istamd flap. We used the refrograde -follow flaps repairing the tissue defect of procimal leg 6cases and 51cases of lower leg and foot were used by the reverse flaps. Results 54 cases were survived completely,the latter part of flaps were necrotic in 3 cases,but the subcutaneous tissue under the necroseis were alive and healed by skin graft, by follow-up,the terture,appearance and function of the flaps were good. Conclusions The sural neurovascular flap has sufficient blood supply and been easily transferred, It can be used to repair the tissue defect around the leg, ankle and foot.
