CT引导下臭氧联合胶原酶治疗腰椎间盘突出症的临床观察

目的 观察CT引导下经皮穿刺注射臭氧联合胶原酶化学溶解术治疗腰椎间盘突出症的临床疗效及安全性.方法 通过CT扫描精确定位,将穿刺针准确刺入靶点,推注40~60 μg/ml的臭氧5~20 ml和600~1200 U胶原酶,分别观察治疗后1个月和6个月的临床疗效及术后并发症,进行疗效和安全性评价.结果 术后1个月:优42例(58.33%);良21例(29.17%);可5例(6.94%);差4例(5.56%),优良率为88.50%.术后6个月:优45例(62.50%);良24例(33.33%);可3例(4.17%),优良率达95.83%.术后出现过敏反应、神经系统症状各2例,感染1例,经对症治疗后,均缓解或消失.结论 CT引导下经皮穿刺注射臭氧联合胶原酶化学溶解术治疗腰椎间盘突出症疗效确切、微创、安全,值得推广应用.

The observation of clinical efficacy and safety of CT-guided percutaneous injection of medical ozone combined collagenase chemonucleolysis (CCN) in the treatment of lumbar disc herniation

Objective To observe the clinical efficacy and safety of CT-guided percutaneous injection of medical ozone combined collagenase chemonucleolysis(CCN) in the treatment of lumbar disc herniation.Methods The needle were pierced accurately to the targets by precise positioning of the CT scan,40~60μg/ml ozone was injected 5 to 20mL and collagenase 600 to 1200 U,the clinical efficacy and postoperative complications were observed one month and 6 months after treatment to evaluate the efficacy and safety.Results After one month:excellent in 42 cases(58.33%);good in 21 cases(29.17%);mild in 5 cases(6.94%);and poor in 4 cases(5.56%),the excellent rate was 88 50%,6 months after surgery:excellent in 45 cases(62.50%);good in 24 cases(33.33%);mild in 3 cases(4.17%),the excellent rate was 95.83%.Two cases allergic reaction,two cases neurological symptoms and one case infectionwere observed,but after symptomatic treatment,all Symptoms were relieved or disappeared.Conclusion CT-guided percutaneous injection of medical ozone combined collagenase chemonucleolysis(CCN) is effective and safety in the treatment of lumbar disc herniation.
