新生儿窒息后血糖变化及干预研究

摘    要:目的 探讨窒息新生儿的血糖变化及其干预方法.方法 采用微量血糖测定仪检测110例窒息新生儿入院时的血糖,对合并低血糖和高血糖的患儿进行动态血糖监测,并通过调整葡萄糖的输注速度和浓度,使血糖控制在正常范围.结果 110例窒息新生儿入院时检出低血糖42例(占38.2%),高血糖26例(23.6%).低血糖组24 h时血糖基本恢复正常.高血糖组24 h后血糖有所下降,但未恢复正常,继续监测至48 h时血糖均值在正常范围,其中死亡2例临终前血糖17 mmol/L.结论 对窒息新生儿应加强血糖动态监测,及时纠正窒息引起的糖代谢紊乱有助于改善患儿预后.

Abstract:Objective To investigate the change of blood glucose and its intervention in newborns with asphyxia,Methods The blood glucose levels of 110 newborns with asphyxia were detected at admission by blood sugar microanalysis instrument,Newborns with hypoglycemia or hyperglycemia were continuously blood glucose monitored,and kept blood glucose in normal range by adjusting the velocity and concentration of intravenous infusion of glucose.Results 42 asphyxia newborns(occupied 38.2%) with hypoglycemia and 26 cases(23.6%) with hyperglycemia in 110 cases at admission,The blood sugar of the hypoglycemia group enhanced to normal range after 24 hours of admission,The blood sugar of the hyperglycemia group dropped after 24 hours and the mean blood sugar lowered to normal range after 48 hours of admission,the blood glucose levels before died more than 17 mmol/L in 2 cases of death,Conclusion The continuous blood glucose monitoring should be strengthened in asphyxia newborns,early treatment of glucose metabolic disorder caused by asphyxia can improve the prognosis.
