十二指肠间质瘤的CT诊断

目的 探讨十二指肠间质瘤的CT表现及诊断价值.方法 回顾性分析手术或病理证实的16例十二指肠间质瘤患者的CT平扫及增强扫描资料.结果 16例十二指肠间质瘤中降部9例,水平部5例,升部2例.良性间质瘤6例,均向腔内生长,呈圆形或类圆形,密度均匀,边界清楚.恶性间质瘤7例,向腔外生长6例、腔内外生长1例,肿瘤边缘有分叶,密度不均匀,明显不均匀强化6例,均匀强化1例,周围组织侵犯或远处转移5例.潜在恶性3例,均向腔内外生长,密度均匀.CT检查的敏感度为100%,定位准确率为93.8%,定性准确率为75%.结论 CT平扫及强化能准确显示十二指肠间质瘤的部位、形态、大小及其血供,对鉴别肿瘤良恶性有重要临床价值.

Diagnostic value of helical CT in duodenal stromal tumors

Objective To investigate the CT features and to evaluate the diagnostic value of helical CT in duodenal stromal tumors.Methods CT features of 16 patients with surgically and pathologically proved duodenal stromal tumors were retrospectively analyzed,Plain and enhanced CT were performed in all 16 cases.Results Of the 16 cases,9 lesions were located in descending part,5 in horizontal part,2 in ascending part.6 cases were benign tumors,all were grew toward in cavity and mainly round or oval in shape.7 cases were malignant,6 grown outward cavity and 1 in-out cavity,all were lobular in shape and extensively heterogeneous enhancement but one.Invasion and metastasis of adjacent structures(5 cases).3 cases were tumors tend,all were grew toward in cavity and homogeneous density.The sensitivity,accuracy for location and quality of CT were 100%,93.8%(15/16),75%(12/16).Conclusion Plain and enhancement CT scan have great value in diagnosing duodenal stromal tumors which can show the situation,shape,size and artery of the tumor clearly.
