腔镜手术序贯131I消融治疗甲状腺癌的可行性研究

摘    要:目的探讨腔镜手术序贯131I消融治疗甲状腺癌的可行性。方法回顾性分析2008年2月至2011年10月我院收治39例低危甲状腺癌患者临床资料,其中行腔镜手术序贯131I消融治疗19例(腔镜组),行传统开放手术治疗20例(传统组),统计学分析两组间手术时间、术中出血量、术后引流量、术后肿瘤种植及复发转移率、患者切口满意度和并发症发生率等指标的差异。结果腔镜组19例患者均成功完成手术。腔镜组术后患者满意度高于传统组,有显著性统计学差异(P<0.01);而手术时间、术中出血量、术后引流量、术后并发症、术后肿瘤种植及复发转移,两组间无统计学差异(P>0.05)。结论腔镜手术序贯131I消融治疗低危的分化型甲状腺癌安全、可行。

Study on the feasibility of laparoscopy followed by radical operation for thyroid cancer patients

Abstract:Objective Evaluating the feasibility of laparoscopy by approach of chest wall and mammary areola followed by radical operation for thyroid cancer patients.Methods To retrospectively analyze the clinical data of 39 cases diagnosed as early papillary thyroid carcinoma,which included 19 cases treated with laparoscopy followed by radical operation and 20 cases with traditional open operation.To compare the operating time,amount of bleeding on operation,drainage after operation and the different ratio of patient satisfaction and tumor recurrence between the two groups.Results It was successful for 19 cases treated with laparoscopy followed by radical operation and the ratio of their satisfaction with treatment was higher than the patients in the group with traditional open operation(P<0.01).It had no difference on the operating time,amount of bleeding on operation,drainage after operation and the ratio of tumor recurrence statistically(P>0.05).Conclusion It is reliable and feasible with treatment of laparoscopy by approach of chest wall and mammary areola followed by radical operation to some cases strictly screened with low-risk early papillary thyroid cancer.
