全喉切除患者围术期心理变化的分析与护理干预效果

目的 分析全喉切除患者围术期的心理变化并提出心理行为干预。方法 对象为我院2003年1月至2005年4月间收治的喉癌患者51例。在全喉切除围术期采用症状自评量表（SCL-90）进行评估并采取心理干预。结果 症状自评量表SCL-90表明：全喉切除患者术前各因子分，除敌对、偏执症状外均高于全国常模，患者存在较严重的不良心理症状。而术后2周的评估各因子分与术前比较。结果均低于术前。具有非常显著性差异（P〈0．01），除强迫、人际敏感、抑郁、焦虑．恐怖等症状无明显差异外．其余均显著低于全国常模。结论 重视全喉切除患者围术期存在的不良心理症状，采取积极的有针对性的心理干预措施，对其躯体疾病的康复治疗进程及生存质量的提高有积极的促进作用。

Nursing Interventions and Investigation of Mental Status of Perioperative Patients with Total Laryngectomy

Objective To investigate the mental status and the influence of mental nursing interventions on the perioperative patients with total laryngectomy. Methods Fifty-one patients with throat tumor in our hospital from January 2003 to April 2005 were evaluated with SCL-90 and given mental interventions during the perioperative periods. Results Before operation, all scores of SCL-90 factors except hostile and paranoid state were higher than national average level. All patients had serious mental symptoms. Two weeks after operation, all symptoms improved obviously except forcement, human relations sensitiveness, depression ,anxiety, and terror. Conclusion It is recommended to pay more attention to body and mind symptoms of the perioperative patients with total laryngectomy and to take specific mental nursing interventions, so as to help the patients to facilitate recovery and improve living quality.
