摘    要:目的:验证湿润烧伤膏(简称MEBO)药物对深度(包括Ⅱ度及Ⅱ度以上)烧伤创面的治疗作用,考察其适用性、安全性、有效性。方法:采用分层抽样调查方法和填表方式调查2000年1月至2005年1月之间,对国内烧伤治疗具有较大影响力的12家三级医院烧伤救治中心开展湿性医疗技术治疗深度烧伤创面的临床资料,通过归纳分析,进一步了解MEBO对不同原因、不同部位、不同深度烧伤创面的愈合方式、治疗痛苦程度、止痛效果、并发症与药物毒副反应等指标的临床疗效。结果:60例病人中成人组平均年龄35.3±9.61岁,平均烧伤面积26.56±16.35%TBSA;小儿组平均年龄2.81±1.60岁,平均烧伤面积27.38±19.91%TBSA。深Ⅱ度深型烧伤创面平均愈合时间25.42±7.43天,Ⅲ度浅型烧伤创面平均愈合时间33.53±7.26天。深Ⅱ度深型瘢痕发生率为9.7%,Ⅲ度浅型45.1%,但它们的瘢痕增生范围与传统疗法比均有缩小,p值均<0.01,同时减少了Ⅲ度创面的植皮面积和次数,缩短了抗菌药物的使用时间,降低了创面和全身感染发生率。结论:MEBO适于各种原因、各种面积、各部位深度烧伤创面的治疗,可防止或减少瘢痕愈合、挛缩和残疾,提高病人生活质量,尤其对Ⅱ度深型和Ⅲ度浅型烧伤创面的治疗具有明显的安全性和有效性。

Abstract:Objective: To verify the therapeutic effect of MEBO in treating deep burn wounds (including 2^nd degree and deeper burn wounds) and to investigate its applicability, safety and efficacy. Method: Stratified sampling method was adopted. The patients with deep burn wounds treated with MERT/MEBO in 12 leading hospitals during the period from Jan. 2000 to Jan. 2005 were asked to fill in investigation forms and their clinical data were collected from the hospitals. Analysis of all the information was done to further verify the effect of MEBO in treating burn wounds of different causes, in different positions and with different depths. The indexes included mode of wound healing, sensation of pain in the treatment and pain relieving effect of MEBO, complications and drug adverse effects. Result: Among the 60 patients, adults had an average age of 35.3±9.61 and an average burn area of 26.56±16.35%TBSA, and children had an average age of 2.81±1.60 and an average burn area of 27.38±19.91% TBSA. The average healing time of deep 2^nd degree burn wounds was 25.42±7.43 days and the average healing time of superficial 3^rd degree burn wounds was 33.53±7.26 days. Deep 2^nd degree burn wounds had a scar formation rate of 9.7% and superficial 3^rd degree burn wounds 45.1%, The area of the scar was smaller than that of conventional treatment, P<0.01. The area and frequency of skin grafting for 3^rd degree burn wounds were reduced, the duration of antibiotic drug administration was shortened and the systemic and local wound infection rate was decreased. Conclusion: MEBO is suitable for treating deep burn wounds of different causes, with different area and in different positions. It can prevent scar formation and contracture and reduce disablement rate, so that the living quality of the patient is improved. MEBO is very safe and has significant efficacy in treating deep 2^nd degree and superficial 3^rd degree burn wounds.
