| 摘 要： | 目的：探讨小肠段代直肠的可行性。方法：取带血管蒂回肠一段，一端回结肠端侧吻合，一端肛管吻合或外置，重建肛门括约肌术15例。结果：15例无吻合口瘘，肠坏死，无手术死亡，13例括约肌成形功能良好。结论：回肠血供丰富，系膜松解后，长度可满足重建直肠，内括约肌成形，技术可行，不增加手术难度，可保证吻合口血运，提高肿瘤根治率，减法术后并发症和局部复发，是理想的自体替代材料。

| Abstract: | Objective: To lengthen colon by improving blood circulation and rebuiding or preserving proctodeum and to reduce complication in radical operation by substituting ileum for rectum Methods: Perform ileocostomy with a section of ileum end and anastomosing the other end with proctodeum Results: 15 case of patients had not anastomotic leakage, necrosis or death during and after operation The function of plastic sphincter was good Conclusion: The blood supply of ileum is ample, after lusis of frena, the ileum length is sufficient for rebuilding rectum, plastic operation for internal sphincter The operation of substituting ileum for rectum is practicable |
