直肠癌根治术后转移的临床特点

目的：探讨影响直肠癌根治术后远处转移的临床特点。方法：回顾性分析中国医学科学院肿瘤医院１９８０年１月～１９９３年１２月直肠癌根治性手术后患者８０１例，对影响转移因素做了统计学处理。结果：术后发生转移患者１５８例，转移部位依次为：肺转移６９例，肝转移及腹股沟淋巴结转移各３４例，术后３年内转移发生率占７５．０％（１１９／１５８）。影响转移因素包括：肿瘤分化程度（Ｐ〈０．０１），Ｄｕｋｅｓ分期（Ｐ〈０．

Metastases after Curative Surgery of Rectal Carcinoma

Objective: To explore the metastases and factors influencing metastases after curative resection of rectal carcinoma. Methods: Eight hundred and one patients with rectal carcinoma who underwent curative resection between Jan. 1980 and Dec. 1993 in Cancer Hospital, were statistically analyzed about metastases. Results: One hundred and fifty - eight patients developed metastasis later after curative resection. The main metastatic sites were 69 cases in lung, 34 cases in liver and as the same as in inguinal node. of the patients with metastases, 75% developed in first three - year after surgery. The factors influencing metastasis include that, grade of tumor (P < 0. 01 ), Dukes stage (P < 0. 001 ); low site of tumor, mucoid and ring cell carcinoma were of higher metastatic rate (P< 0. 05). Most of the inguinal metastases developed in patients with tumor 7 cm or less than 7 cm from anal verge. The 5 - year survival of the patients with metastasis - resection was 28. 6 %. None of the other modality was alive 5 - year. Conclusion: Most of the metastatic sites after curative surgery of rectal carcinoma was lung then liver and inguinal node. Resection of the metastasis was the best modality if the lesion could be excised.
