高血压脑出血血肿周围组织血管内皮生长因子的表达

摘    要:目的探讨高血压脑出血患者血肿周围组织血管内皮生长因子的表达及其与血肿周围脑水肿形成的关系.方法取34例脑出血患者行微创血肿抽吸术,抽吸过程中获得的血肿周围脑组织,采用免疫组织化学技术测定其血管内皮生长因子(VEGF)的表达;根据术前头颅CT测量脑出血患者血肿周围脑水肿带的大小,用双盲法对染色结果及术前脑水肿的体积进行分析.结果脑出血后4 h,血肿周围组织已出现VEGF的表达,脑出血后48~72 h,VEGF的表达达到高峰.VEGF表达强阳性组、弱阳性组、阴性组之间比较,脑水肿体积的差异有显著意义(P<0.01).结论 VEGF在高血压脑出血血肿周围组织中表达上调,可能参与了血肿周围脑水肿的形成.

Abstract:Objective To investigate the expression of vascular endothelial growth factor(VEGF) in perihematomal brain tissue and its correlation with formation of brain edema in patients with hypertensive cerebral hemorrhage.Methods Expressions of VEGF in perihematomal brain issue which collected in the course of minimally invasive surgery was detected by imunohistochemical techniques in 34 patients with hypertensive cerebral hemorrhage.The volume of perihematomal brain edema on computed tomographic scan was determined before surgery.The results of staining and the volume of perihematomal brain edema were analyzed in double blind fashion.Results expression of VEGF in perihematomal brain tissue existed at 4 h and reached peak at 48~72 h after cerebral hemorrhage.There were obvious difference in volume of perihematomal brain edema with different expression of VEGF in perihematomal brain tissue (P<0.01).Conclusion The expression of VEGF is upregulated in perhematomal brain tissue in patients with hypertensive cerebral hemorrhage,suggesting that VEGF may participate in formation of perihematomal brain edema.
