消化管支架在治疗消化管良恶性狭窄中的应用

目的探讨消化管金属、塑料支架在治疗消化管良、恶性狭窄中的治疗效果和安全性.方法对38例恶性食管、胃出口、大肠、胆管狭窄和9例良性胆管、胰管狭窄的患者,行狭窄扩张术后,在内镜直视或内镜X线配合下,置入金属或塑料支架,观察其对狭窄的治疗效果及并发症.结果47例患者均顺利置放支架,消化管梗阻的症状得到缓解,主要并发症是支架置放后短期内轻度疼痛.结论金属支架置放治疗能明显改善晚期消化管癌症患者的恶性梗阻症状,提高生存质量,延长生存时间,塑料支架置放治疗对胆胰管良性狭窄具有明显治疗效果.

Application of digestive tract stent therapy on benign and malignant stricture of digestive duct

Objective To investigate the value and security of treating the benign and malignant stricture of digestive duct with plastics or metal stent.Methods After the stricture dilation,38 cases of the malignant stenosis of esophagus,gastric outlet,large intestine,bile duct and 9 cases of the benign stenosis of bile duct,pancreatic duct were inserted metal stent or plastics stent under endoscopic guidance or endoscope and X-line guidance.The clinical value and complications of stent therapy on the strictures were observed during the study period.Results The stent was placed successfully in all of the 47 patients.Immediate relief of the symptoms of digestive tract obstruction was observed.The main complication of this procedure was slight ache in short period.Conclusion Inserting metal stent therapy on the malignant stenosis results in marked improvement of the patient's obstruction symptoms,living quality and life span.Inserting plastics stent therapy on the benign stenosis of bile-pancreas duct has marked treatment value.
