颅脑外伤并发颈性眩晕的早期影像学诊断意义

摘    要:目的评估颈椎X线和经颅多普勒(TCD)对颅脑外伤并发颈性眩晕的早期诊断价值。方法对256例轻、中型颅脑外伤患者,早期分别行颈椎X线和TCD检查,用Logistic回归分析,了解二者与发生颈性眩晕的关系。结果256例中,并发颈性眩晕69例。颈椎X线异常114例(44.5%),TCD示椎一基动脉供血不足201例(78.5%)。Logistic回归分析提示二者均有统计学意义。结论颅脑外伤并发颈性眩晕与颈椎X线和TCD异常有密切关系。早期行颅脑外伤颈椎X线和TCD检查,对颈源性眩晕的诊断有重要参考价值。

Abstract:Objective To evaluate the value of cervical spine X-ray and transcranical ultrasound doppler (TCD) for early diagnosis of cervical vertigo in head injury. Methods 256 cases suffering from mild or middle head injury were examined by cervical spine X-ray and TCD. Analyse the relation between abnormity of their results and cervical vertigo. Results Of 256 patients, 69 cases had cervical vertigo, cervical spine X-ray showed abnormal in 114 cases (44.5%),and TCD displayed insufficient blood in the vertebrae-base artiries in 201 cases (78.5%). Logistic regression amalysis indicated two factors had statistic meaning. Conclusions There is obvious correlative between cervical vertigo in head imjury and abnormity of their cervical spine X-ray and TCD. Cervical spine X-ray and TCD have important value for the early diagnosis of cervical vertigo.
