痛风性肾病的早期诊断及其相关因素分析

摘    要:目的 探讨尿酸结石患者尿液微量白蛋白 (ALB)、转铁蛋白 (TRF)、视黄醇结合蛋白 (RBP)和N 乙酰 β D 氨基葡萄糖苷酶 (NAG)变化与早期痛风肾病的关系。方法 用酶联免疫法测定 89例尿酸结石患者尿液ALB ,TRF ,RBP含量 ,同时测定尿NAG浓度 ,并对伴发因素做相关性分析。结果 尿酸结石组尿ALB ,TRF ,RBP ,NAG均较正常对照组显著增高 ,且与病人年龄、病程、血尿酸水平及伴发肥胖、高脂血症、糖尿病、高血压的发病密切相关。结论 尿酸结石患者临床尿蛋白阴性和血尿素氮 (BUN)、肌酐 (Cr)正常时 ,肾功能已有损伤 ,尿ALB ,TRF ,RBP ,NAG检测可作为痛风肾病早期诊断的敏感指标

Abstract:Objective To study the change of urine micro albumin(ALB), micro trarsferrin(TRF), retinol binding protein(RBP), N acetyl beta D glucosaminiduse(NAG)and the relationship between their concentration and the early nephropathy in patients with urinacidistone Method We measured the concentration of ALB, TRF, RBP in the urine of 89 patients with urinacidistone by ELISA, and the urinary NAG was also assayed Result The urinary TRF, RBP, NAG in patients with urinacidistone were significantly higher than those in control group( P <0.01), they related positively to each other There was a close correlation among those urinary measured concentration, blood urinacid, age and duration of patients with urinacidistone, fat, diabetes, hyper lipemia, hypertension Conclution Renal function has been damaged in the early pationts with urinacidistone, the urine ALB, TRF, RBP, NAG are initial indicatrix to predict gouty nephropathy
