内窥镜下会师治疗后尿道闭锁疗效分析

目的探讨内窥镜下会师治疗后尿道闭锁的手术技巧及临床治疗。方法 56例入院前均已行耻骨上膀胱穿刺造瘘术的后尿道闭锁患者,闭锁段长度0.6～2.8cm,平均1.7cm,麻醉下分别从尿道及膀胱造瘘口置入尿道冷刀切开镜,会师后行尿道内切开术及瘢痕电切术,术后留置尿管1个月。结果 56例拔出尿管后均排尿通畅,随访1年,13例术后3个月后不同程度尿线变细,行尿道扩张3~5次后症状消失。结论内窥镜下会师治疗后尿道闭锁是一种简单有效的治疗方法,创伤小,手术成功率高,可在临床上推广。

Therapeutic efficiency of posterior urethratresia treated with endourethral meeting surgery

Objective To assess the therapeutic efficiency of posterior urethratresia treated with endourethral meeting surgery.Methods 56 patients with posterior urethratresia were performed suprapubic puncture,and the stricture length ranged from 0.6 to 2.8 cm,averaging at 1.7 cm.All surgeons were divided into two groups:one group operated under urethral endoscope,,the other group operated by orificium fistulae of bladder.After they meeting,internal urethrotomy and cicatrix electrotomy were performed.The urinary catheter was detained one month.Results Among all the 56 successful operations,had been followed up 1 years,and stricture recurred in 13 patients,symptom disappear by performed urethral sounding three to five times.Conclusion It is effective to treat posterior urethratresia by endourethral meeting surgery,with the virtues of micro-trauma and high operation achievement ratio.
