阿托品预防老年冠状动脉支架植入术后拔鞘管致心血管迷走反射的作用

目的：探讨阿托品对老年患者冠状动脉支架植入术后拔鞘管致心血管迷走反射的预防作用。方法：回顾性分析行冠状动脉造影检查及支架植入术老年患者300例的临床资料，根据拔管前是否应用阿托品分为观察组和对照组，每组150例。观察组患者在拔除鞘管前10min皮下注射阿托品0．5mg，对照组未注射阿托品而直接拔除鞘管。比较两组迷走反射发生率。结果：观察组发生血管迷走反射2例（1．3%），对照组发生12例（8．0％）。两组迷走反射的发生率差异有显著性（X^2=7．49，P〈0．01）。结论：冠状动脉支架植入术后的患者在拔除鞘管前10min皮下注射阿托品0．5mg可有效降低血管迷走反射的发生率。

Effect of atropine on preventing cardiovascular vagal reaction at removing sheath in coronary artery stent implantation in elderly patients

Objective： To investigate the protective effect of atropine on the vagal reaction at removing the sheath in coronary artery stent implantation. Methods：Clinical data of 300 patients undergoing coronary artery stent implantation were analyzed. They were divided into prevention group （150 cases） and control group （150 cases） according to the treatment with or without atropine before sheath removal in intervention therapy. In prevention group, 0.5 mg atropine was subcutaneous injected 10 minutes before removing sheath,but no atropine was used in control group. The incidence of vagal reaction was compared between the two groups. Results： Two patients showed cardiovascular vagal reaction in prevention group （1.3%）, while 12 cases did in control group （8.0%）, and there was a significant difference between the two groups （X^2 = 7.49 ,P〈0.01）. Conclusion：The method of atropine subcutaneous injection 10 minutes before removing sheath in coronary artery stent implantation was helpful to reduce the incidence of cardiovascular vagal reaction.
