瑞芬太尼复合异丙酚在声带息肉摘除术麻醉中的应用

摘    要:目的研究瑞芬太尼复合异丙酚在声带息肉手术麻醉中的应用。方法选择60例。ASAⅠ-Ⅱ级无合并症,择期行声带息肉摘除患者。随机分为观察组和对照组.每组各30例。对照组用异丙酚-芬太尼诱导和七氧烷吸入维持麻醉;观察组用异丙酚.瑞芬太尼诱导和维持麻醉。观察两组麻醉诱导和维持期间血流动力学变化,术毕停药后病人自主呼吸恢复时间、呼之睁眼时间、拔管时间和不良反应等。结果观察组麻醉中循环波动小,病人自主呼吸恢复快.呼之睁眼时间,拔管时间明显较对照组快。两组不良反应无统计学意义(P〉0.01)。结论瑞芬太尼复合异丙酚可有效安全地用于声带息肉手术麻醉。

Abstract:OBJECTIVE To study the application of remifentanil as supplement to propofol anesthesia in polyp of vocal cord extirpation. METHODS 60 patients(ASAⅠ-Ⅱ ) undegoing polyp of vocal cord extirpation were randomly divided into two groups(n= 30), observation group were given anesthetic induction and maintenance with propofol-remifentanil; while control group were given anesthetic induction with propofol-fentanil and maintenace with sevoflurace. The lemodynamics during induction of anesthesia and of maintenance anesthesia, and time for stopping administration of anesthetic until full spontaneous respiration, eye opening, tracheal extuhation, orientation and discharging from the postanesthetic care unit (PACU) were compared. RESULTS Anesthesia by remifentanil produces less circulatory fluctuation, and less time for full spontaneous respiration, eye opening, tracheal extubation, orientation and discharging from the PACU than contral group. CONCLUSION Remifentanil can be used safely and effectively in polyp of vocal cord extirpation.
