足穴按摩对糖尿病足高危人群干预作用的临床研究

目的：观察常规基础治疗之上,足部穴位按摩对具有糖尿病足高危因素患者的下肢血流状态、神经传导功能的干预效果。方法：随机、对照临床研究,试验组20例予糖尿病基础治疗＋足部穴位按摩,对照组20例予糖尿病基础治疗,共治疗12周,观察两组临床总疗效、足部感觉异常（VAS评分）、神经电生理检测、踝肱动脉压比值（ABI）改善情况的变化。结果：足穴按摩组在改善肢端麻木感、疼痛感、冷感、感觉减退诸症和神经传导速度及诱发电位波幅、踝肱比指数等方面明显优于对照组（P〈0.01）,实验组临床总有效率（90%）,明显高于对照组（30%）,差异有显著性（P〈0.01）。结论：足部穴位按摩对糖尿病足早期神经病变、下肢血管病变的血流状态及临床症状有一定改善作用,可降低糖尿病足发生的危险因素,减少足溃疡的发生,值得临床推广。

Clinical Research into the Intervention Effect of Foot Acupoint Massage on the Patients with High-risk Diabetic Foot

Objective：To investigate the therapeutic effect of foot acupoints massage combined with the regular treatment on the bloodstream status and nerve conduction of the patients with high risk of diabetic foot.Methods：In the randomized control clinical study,40 diabetics were divided into the basic therapy group（20 subjects receiving regular treatment） and combined therapy（20 subjects receiving foot acupoints massage based on the regular treatment）.The curative effect,foot paraesthesia（VAS score）,neuroelectricity examination,ankle-brachial index of the two cohorts were evaluated before and after 12 weeks of therapy.Results：The acroanesthesia,soreness,cold sensation,hypoesthesia,nerve conduction velocity and the amplitude of evoked potential in combined therapy group were significantly improved,as compared with the control group（P0.01）.Furthermore,its total effective rate was significantly higher than that of the basic therapy group（85% vs 30%,P0.01）.Conclusion：Foot acupoints massage could improve the bloodstream status and clinical symptom of diabetic neuropathy at the early stage.The therapy should be generalized in clinical practice for its role in decreasing the risk of diabetic foot and lessoning ulcer occurrence.
