无创呼吸机治疗老年慢性阻塞性肺疾病急性加重期合并呼吸衰竭的效果评价

目的探究无创呼吸机双水平无创正压通气(BiPAP)治疗老年慢性阻塞性肺疾病急性加重期(AECOPD)合并呼吸衰竭的临床治疗效果。方法将本院收治的62例AECOPD合并呼吸衰竭的老年患者随机分为实验组与对照组,两组患者均给予解痉、平喘、氧疗、抗感染、纠正酸碱电解质平衡紊乱等呼吸衰竭的常规治疗措施,实验组在此基础上增加BiPAP辅助治疗。治疗完成后比较两组患者的动脉血气指标,发绀、呼吸困难、心跳过速等症状缓解率。结果实验组患者在治疗后血气指标显著优于对照组,临床症状的缓解率显著高于对照组,差异具有统计学意义(P<0.05)。结论对于AECOPD合并呼吸衰竭的老年患者,在常规治疗的基础上增加无创呼吸机的辅助治疗,临床疗效显著,方便实用,值得临床推广应用。

Effect Evaluation of Non-invasive Ventilator Treatment in Elderly AECOPD Patients with Respiratory Failure

Objective To explore the clinical effect of bi-level positive airway pressure（BiPAP） with non-invasive ventilator treating the elderly chronic obstructive pulmonary disease with acute exacerbation（AECOPD） patients complicated by respiratory failure.Methods 62 cases of AECOPD with respiratory failure in our hospital were randomly divided into experimental and control groups.Both groups were treated with conventional treatment such as antispasmodic measures,antiasthma measures,oxygen therapy,anti-infective measures,correcting balance disorders measures.The experimental group was given the BiPAP treatment in addition.Then compare arterial blood gas index,cyanosis,dyspnea,tachycardia and other symptoms remission rates between two groups.Results Blood gas analysis and the clinical symptoms of the experimental group patients was significantly better than the control group after treatment（P〈0.05）.Conclusion on the basis of conventional treatment,non-invasive ventilator treatment for elderly AECOPD patients complicated by respiratory failure has a significant clinical effect,and it is convenient,practical,worthy of clinical application.
