下丘脑错构瘤的影像学诊断

摘    要:目的探讨下丘脑错构瘤的影像学表现。方法回顾性分析20例经病理证实的下丘脑错构瘤CT、MR表现。结果 CT表现为鞍上池、脚间池内均匀等密度占位,MR表现为鞍上下丘脑区均匀等皮信号影,与灰结节和乳头体相连,T1WI为等信号,T2WI为稍高信号。CT及MR增强扫描均未见强化。临床主要表现为性早熟和(或)痴笑样癫痫。结论如患儿表现为性早熟或痴笑样癫痫,CT及MR检查发现鞍上下丘脑区等密度或等信号占位,增强无强化,应首先考虑为下丘脑错构瘤。本病应与其它鞍区肿瘤鉴别。

The Imaging Features of Hypothalamic Neuronal Hamartomas

Abstract:Objective to study the finding of imaging features to hypothalamic neuronal hamartomas. Methods study the imaging features of CT and MRI to 20 patients hypothalamic neuronal hamartomas confirnled by pathology. Results the main clinical features of the hypothalamic hamartoma were duo to precocious puberty, and (or) gelastic epilepsy and mental retardation. All mass appears as isodensity and locateded in the suprasellar and inter peduncular cistern on CT. On MRI, the mass was adqulis isointensity to gray matter on locateded in the suprasellar and were closely related to the tuber cinereum or mamillary bodies. The masses was adqulis isointensity on T~WI and slightly hyperintense on T2WI. All masses were not enhanced after enhancement scans on CT and M1KI. Conclusion In children presenting with precocious puberty or gelastic epilepsy,CT and MRI revealed an isointense mass and located in the interpeduncular cistern without enhancement,The diagnosis of hypothalamic hamartoma might be considered fist. It should be differentiated from the other mass suprasellar area.
