核医学显像(SPECT-CT)鉴别头颈部可疑异位甲状腺的价值

摘    要:目的探讨核医学显像鉴别头颈部可疑异位甲状腺的价值,为临床提供参考。方法选择52例到我院接受临床检查并发现甲状腺可疑来源病灶患者,分别采用B超、CT、131I-WBI和SPECT-CT进行检查,以淋巴穿刺取活检和随访6个月联合确定病灶为标准,进行对比分析核医学显像鉴别头颈部可疑异位甲状腺的价值。结果活检和随访6个月联合检出病灶66个,B超检出病灶53个,CT检出病灶58个,131I-WBI检出病灶56个,SPECT-CT检出病灶63个;SPECT-CT灵敏度为98.75%,明显高于B超特异性80.98%、CT特异性85.35%和131I-WBI特异性91.05%,差异具有统计学意义(P0.05);SPECT-CT准确性为96.32%,明显高于B超特异性81.88%、CT特异性83.58%和131I-WBI特异性90.72%,差异具有统计学意义(P0.05);SPECTCT特异性为97.75%,明显高于B超特异性80.22%、CT特异性90.32%和131I-WBI特异性92.28%,差异具有统计学意义(P0.05)。结论 SPECT/CT技术将不断完善,并充分发挥核医学和CT成像技术的优点,为异位甲状腺类疾病提供可靠的鉴定方法。

The Value of Single Photon Emission Computed Tomography on Diagnosis of Head and Neck Ectopic Thyroid

Abstract:Objective To investigate the value of single photon emission computed tomography on diagnosis of head and neck ectopic thyroid, and to provide the reference for clinical. Methods 52 patients were selected in our hospital and the B ultrasound, CT, 131I-WBI and SPECT-CT were examined, respectively. With lymph node puncture biopsy and followed up for 6 months to determine lesion as the standard, carries on the contrast analysis of suspicious ectopic thyroid nuclear medicine imaging in the differential value of head and neck. Results The biopsy and follow-up 6 months combined lesions were detected in 66, B ultrasound detected 53 lesions, CT lesions were detected in 58, 131I-WBI detected 56 lesions, SPECT-CT lesions were detected in 63; the sensitivity of SPECT-CT was 98.75%, significantly higher than B ultrasound 80.98% specificity, 85.35%CT specific and 131I-WBI specific 91.05%, the difference was statistically significant (P〈0.05);the accuracy of SPECT-CT was 96.32%, significantly higher than B ultrasound specificity 81.88%, the specificity of CT 83.58% and 131I-WBI 90.72% specificity, the difference was statistically significant (P〈0.05); SPECT-CT specificity was 97.75%, significantly higher than B Ultrasound specificity 80.22%, the specificity of CT 90.32%and 131I-WBI 92.28% specificity, statistically significant differences (P〈0.05). Conclusion SPECT/CT technology will continue to improve, and fully play the advantages of nuclear medicine and CT imaging technology, providing reliable identification methods for ectopic thyroid diseases.
