胺碘酮治疗不稳定型心绞痛并发阵发性心房纤颤的疗效及对P波离散度和C反应蛋白的影响研究

摘    要:目的:探讨胺碘酮治疗不稳定型心绞痛并发阵发性心房纤颤的疗效及对P波离散度和C反应蛋白的影响。方法选取2010—2013年我院收治的不稳定型心绞痛并发阵发性心房纤颤患者90例,随机分成治疗组和对照组,每组45例。对照组患者进行常规抗血小板聚集、扩张冠状动脉等治疗;治疗组在此基础上加服胺碘酮,两组患者均连续治疗15d。比较两组患者临床疗效,治疗前、治疗15d后P波离散度及C反应蛋白。结果治疗组患者有效率(77.8%)高于对照组(46.7%),差异有统计学意义( P〈0.05)。两组患者治疗前P波离散度、C反应蛋白比较,差异无统计学意义( P〉0.05);治疗组治疗15d后P波离散度与C反应蛋白均低于对照组,差异有统计学意义(P〈0.05)。线性回归分析结果显示,P波离散度与C反应蛋白呈正相关(r=0.62,P〈0.001)。结论胺碘酮治疗不稳定型心绞痛并发阵发性心房纤颤患者疗效显著,能有效降低P波离散度及C反应蛋白。

Abstract:Objective To explore the effect of amiodarone in the treatment of unstable angina patients complicated with paroxysmal atrial fibrillation and its impact on P-wave dispersion and C-reactive protein. Methods A total of 90 unsta-ble angina patients complicated with and atrial fibrillation admitted to our hospital from 2010 to 2013 were selected and randomly divided into treatment group and control group,45 cases in each group. Control group given conventional anti platelet aggregation therapy and dilate coronary artery therapy,while treatment group given amiodarone based on the treatment of control group. Both groups treated for 15 days. Clinical effect,P-wave dispersion,C-reactive protein before and after treatment of the two groups were compared. Results The effective rate of treatment group(77. 8%)was higher than that of control group(46. 7%),the difference was statistically significant(P〈0. 05). There wwas no statistical significances of P-wave dispersion and C-reac-tive protein between two groups before treatment( P〉0. 05);while those of treatment group were lower than those of control group after treatment,the difference was statistically significant(P〈0. 05). Linear regression analysis showed that,P-wave dispersion was correlated with C-reactive protein(r=0. 62,P〈0. 001). Conclusion Amiodarone is effective in the treat-ment of unstable angina patients complicated with atrial fibrillation, which can effectively reduce the P-wave dispersion and C-reactive protein.
