急性髓细胞白血病CD34抗原表达及其临床意义

目的检测CD34抗原在急性髓细胞白血病(AML)中的表达,探讨其与某些临床特征及疗效的关系.方法采用流式细胞术检测AML患者骨髓白血病细胞CD34抗原及髓系与淋系抗原的表达.结果53例初治AML中CD34阳性表达22例,占41.5%.CD34抗原表达与FAB分型无明显相关,但在M3亚型中表达明显低于其它亚型.其它各髓系抗原表达在CD34+组与CD34-组无显著性差异,淋系抗原表达CD34+组明显高于CD34-组.染色体核型异常两组间无显著性差异.CD34+组与CD34-组在年龄、性别、初诊时血红蛋白浓度、白细胞与血小板计数、肝脾淋巴结肿大等均无明显性差异.CD34+组完全缓解率为40.91%,明显低于CD34-组的67.75%.结论CD34+AML对常规化疗效果差,检测CDa4抗原对预测AML疗效具有重要价值.

The significance of CD34antigen expression in acute myeloid leukemia

Objectives To investigate the expression of CD34 antigen in acute myeloid leukemia (AMD and its relationship with the clinical presentations and the effects of treatment. Methods Flow cytometry (FCM) was applied to investigate the expression of CD34 antigen, myeloid antigen and lymphoid antigen on leukemia cell in bone marrow of patients with AML. Results 22 in 53 patients of untreated AML were positive on CD34 antigen expression. There was no relationship between the CD34 expression and FAB subtypes. CD34 expression was significantly decreased in M3 subtypes (2/11). There was no significant difference in the clinical feature and chromosome abnormality between CD34+ and CD34- patients. The complete remission rate of CD34+ AML was 40. 91%, which was lower than that of CD34- AML(67. 75%). Conclusions Detection of CD34expression is helpful in predicting the effect of treatment for AML.
