脑出血患者血清S-100β蛋白和脑氧代谢的监测

摘    要:目的探讨脑出血患者血清S-100β蛋白含量和脑氧代谢指标颈内静脉血氧饱和度(Sjv(O2))和脑氧摄取率(CEO2)的动态变化。方法采用酶联免疫吸附试验双抗体夹心法测定38例脑出血患者在发病后的第1天、第3天及第5天的血清S-100β蛋白含量,与脑出血量及神经功能缺损评分进行相关分析。并对其中18例患者的Sjv(O2)和CEO2进行观察。结果脑出血后的第1天血清S-100β蛋白含量高于对照组(P<0.05),第3天、第5天分别与对照组比较均无显著性差异(P>0.05)。脑出血后的第1天血清S-100β蛋白含量与出血量和神经功能缺损评分相关(P均<0.05)。脑出血后的第1天Sjv(O2)低于对照组(P<0.05),CEO2高于对照组(P<0.05);第3天Sjv(O2)高于对照组(P<0.05),CEO2低于对照组(P<0.05);第5天与对照组比较均无显著性差异(P>0.05)。结论血清S-100β蛋白、Sjv(O2)和CEO2联合监测对判断脑出血病情和预后有一定指导意义。

Abstract:Objective It is to explore the serial changes of the content of serum S-100β protein and indexes of cerebral oxygen metabolism such as Sjv(O2) and CEO2 in patients with cerebral hemorrhage.Methods The serum levels of S-100β protein in 38 patients with cerebral hemorrhage were measured by ELISA at day 1,3 and at day 5.And at the same time,the correlation between the serum levels of S-100β protein in patients with hemorrhage volume and index of brain damage were studied.The Sjv(O2) and CEO2 in 18 patients with cerebral hemorrhage were measured.Results The serum S-100β protein level was higher than that in control group at the first day(P<0.05).But there were no significant differences between the two groups at day 3 and day 5(P>0.05).At day 1,the serum S-100β protein level was correlated with the hemorrhage volume and index of brain damage(P all<0.05).At the first day the Sjv(O2) was lower and CEO2 was higher than those in the control group(P<0.05).At the third day the Sjv(O2) was higher and CEO2 was lower than those in the control group(P<0.05).There was no obvious difference between the two groups at day 5(P>0.05).Conclusion The serum S-100β protein level,Sjv(O2) and CEO2 of cerebral hemorrhage can be an index for predicting the seriousness and prognosis of cerebral hemorrhage.
