粥样硬化性肾动脉狭窄肾内血流参数分析

目的评价肾内叶间动脉彩色多普勒超声参数对动脉粥样硬化性肾动脉狭窄诊断的准确性。方法以肾动脉造影结果为金标准,双盲对照,专人操作,对123例患者共246个肾进行彩色多普勒超声检查。每例患者均测量肾叶间动脉加速时间、加速度、阻力指数。评价肾内彩色多普勒超声参数诊断粥样硬化性肾动脉狭窄的准确性。结果对≥75%动脉粥样硬化性肾动脉狭窄,肾内指标以加速时间为最好,加速度时间>0.07s诊断的敏感性和特异性分别为85.3%及99.0%;加速度次之,频谱显示及测量精确时,加速度<2m/s2诊断的敏感性和特异性分别为82.4%及85.2%。结论肾内动脉彩色多普勒超声参数诊断≥75%粥样硬化性肾动脉狭窄是一种简便可靠的无创检查手段。

Value of color Doppler ultrasonography of intrarenal artery in diagnosing arteriosclerosis renal artery stenosis

Objective To evaluate the accuracy of color Doppler ultrasonography of interenal artery in diagnosing arteriosclerotic renal artery stenosis. Methods One hundred and twenty-two patients with 246 kidneys were examined by color Doppler ultrasonography and renal artery angiography with double-blinded by one observer and the same mechine. The interlobar artery acceleration time(AT),acceleration(AC),resistant index(RI) were measured. Results For patients with renal artery stenosis more than 75%,the sensitivity and specificity of interlobar artery AT>0.07 s were 85.3% and 99.0%,and those of AC>2 m/s2 were 82.4% and 85.2%,respectively. Conclusions Color Doppler ultrasonography of interenal artery is a reliable and convenient examination for diagnosing severe arteriosclerotic renal artery stenosis.
