微创置管引流与传统保守治疗高血压脑出血的临床成本-效果分析

摘    要:目的:评价微创置管引流方法治疗与传统保守治疗下高血压脑出血的成本-效果。方法:按随机分组法将97例高血压脑出血患者分为两组,治疗组57例,采用微创置管引流治疗(A组);对照组40例,采用传统保守治疗(B组)。运用临床经济学成本-效果分析法(C/E)对A组和B组两种治疗方案进行回顾性分析评价。结果:A、B两组临床治疗优良率分别为89.5%和87.5%,差异无统计学意义(P>0.05);但A组功能改善程度优于B组(P<0.05);以临床总有效率计C/E分别为:A组每治愈1例患者成本为14 500.72元和B组每治愈1例患者成本为20 843.34元,A组临床经济学效益优于B组(P<0.05)。结论:两组临床总有效率相近,但A组在功能改善程度上优于B组,而且费用差异有统计学意义,微创置管引流治疗组在临床经济学上更具优势。

Cost benefit and effect analysis of minimally invasive catheter drainage for hypertensive cerebral hemorrhage:a comparison with traditional conservative treatment

Abstract:Objective To compare the effects and health economic evaluation of traditional conservative treatment with those of minimally invasive catheter drainage for the treatment of hypertensive cerebral hemorrhage,and to evaluate cost-effectiveness values.Methods Ninety-seven patients with hypertensive cerebral hemorrhage with 15 to 40ml hematoma volume were divided into group A,B.Group A(n=57)received minimally invasive catheter drainage while group B(n=40)underwent traditional conservative treatment.The health economic evaluation was determined by using cost-effectiveness analysis.Results Effective rates of group A,group B were 89.5% and 87.5%,respectively(P>0.05),and the improvement of the function in the treatment group was superior to that in the control group(P<0.05),and the cost were 14500.72 yuan,20843.34 yuan,respectively.Conclusion Minimally invasive catheter drainage for the treatment of hypertensive cerebral hemorrhage could significantly contribute to functional recovery of hypertensive cerebral hemorrhage patients and reduce cost remarkably.
