螺旋CT多平面成像在纵隔肿瘤诊断及治疗中的价值

目的：探讨CT多平面成像在纵隔肿瘤诊断及临床治疗中的价值。方法：46例经手术或活检病理证实为纵隔肿瘤患者，行螺旋CT容积增强扫描；图像经后处理获得纵隔肿瘤多平面图像。结果：多平面图像能够全面显示纵隔肿瘤上下范围、长度，肿瘤与周围器官组织的关系。28例良性纵隔肿瘤包括胸腺瘤、畸胎瘤、胸内甲状腺瘤、气管囊肿及神经源性肿瘤，多平面图像示肿瘤形态比较规则，轮廓清楚，与大血管、心脏、胸壁及肺组织之间有清晰的脂肪间隙存在；同时清楚显示肿瘤在纵隔分区内的位置。6例纵隔恶性肿瘤形态不规则，边缘不清楚；可见肿瘤与大血管、心包、胸膜、气管之间间隙消失，肿瘤与之黏连或包绕紧密，呈灌铸状；12例淋巴瘤则见纵隔、肺门处多发肿块。结论：螺旋CT多平面成像可以对纵隔肿瘤生长及有无周围侵犯进行很好观察，对临床手术及放化疗治疗方案的选择有重要价值。

Spiral CT multiplanar reconstruction in mediastinal tumor diagnosis and treatment value

Objective ：To investigate the clinical value of multi -planar CT imaging in mediastinal tumor. Methods ： Forty six cases of surgically or pathologically confirmed mediastinal tumor patients, were performed with enhanced spiral CT scan ; images obtained by the treatment of mediastinal tumor after the multi - planar images. Results： Multiplanar images showed the full scope of mediastinal tumor from top to bottom, .the length of tumor and surrounding organs ,28 cases of benign mediastinal tumors, including thymoma, teratoma, intrathoracic thyroid adenoma, cyst and tra- cheal neurogenic tumor, multi -planar images showed tumor morphology comparison rules, outline clear, with the great vessels, heart, chest wall and lung tissue between the fat with clear gap exists. Shape of 6 cases mediastinum malignant tumor was anomalous,the edge not clear; Obviously between the tumor and the trunk, the pericardium, the pleural membrane,the trachea gap vanishing. Conclusion： Spiral CT multi - planar imaging can show mediastinai tumor clearly and guide clinical surgery, radiotherapy and chemotherapy treatment programs.
