腹腔镜与开放手术行肾切除术的护理

目的探讨腹腔镜肾切除术的护理方法。方法肾切除患者50例，25例采用腹腔镜手术（A组），25例采用开放手术（B组），观察并记录患者术前抑郁与焦虑的心理评分、术后的疗效、术中出血量、术后下床活动时间、术后胃肠功能恢复时间、抗生素应用时间、止痛药应用次数、术后住院时间，住院费用等方面。结果A组在手术中出血量、术后下床活动时间、术后胃肠功能恢复时间、抗生素应用时间、止痛药应用次数、术后住院总天数方面明显优于B组，有显著性差异（P均〈0．05）；B组在术前抑郁与焦虑评分、住院总费用方面优于A组，有显著性差异（P均〈0．05）。结论肾切除运用腹膜后腹腔镜手术较传统经腹膜后开放手术具有临床疗效相当，患者创伤小，痛苦少，恢复快等优点。

Nursing on laparoscopic nepbrectomy and open nephrectomy

Objective To explore the nursing on laparoscopic nephrectomy.Methods 50 patients adopted nephrectomy.Among them,25 cases(group A) received glaparoscopic nephrectomy and 25 cases(group B) received open nephrectomy.The status of depression and anxiety before the operation,the effective treatments after operation,amount of bleeding during operation,the recovery time after the operation,the times of using antibiotic,the times of using analgesic after the operation,total cost of hospitalization and so on were compared between group A and group B.Results Compared with n group B,the amount of bleeding during operation,the recovery time of intestinal function after the operation,ambulation time after the operation,the time of using antibiotic,the times of using analgesic,and the hospitalization days of group A were better(P<0.05).But the scores of depression and anxiety before the operation and medical expense of group B were better than that of group A(P<(0.05).)Conclusions Compared with open nephrectomy,laparoscopic nephrectomy has the same effect and the advantages such as less hurt and pain,rapid recovery.
