奥马哈问题分类系统在社区老年2型糖尿病患者护理问题评估中的应用

目的应用奥马哈问题分类系统发现社区老年2型糖尿病患者护理问题。方法以改良后的奥马哈问题分类系统为评估工具,对在社区医院就诊的180例老年2型糖尿病患者进行护理评估,识别患者存在的护理问题。结果 180例社区老年2型糖尿病患者共存在2 300个护理问题,其中主要发生在健康相关行为领域(42.4%)和生理领域(40.2%)。发生率超过50.0%的护理问题有血糖监测(97.8%)、药物治疗方案(95.6%)、足部护理(95.0%)、营养(94.0%)、循环(92.8%)、联络社区资源(70.0%)、视觉(68.3%)等。结论社区老年2型糖尿病患者在健康相关行为领域以及生理领域存在较多健康问题,护理人员应根据评估结果及时准确地为患者制定针对性的护理计划,采取通俗易懂的健康教育模式,引导患者建立正确的健康相关行为,积极参加社会活动,达到在生理、心理、社会适应上的最佳健康状态。

Application of Omaha classification system in assessment of elderly patients with type 2 diabetes

Objective To identify health care problems in elderly patients with type 2 diabetes through Omaha classification system.Methods With modified Omaha classification system,180 elderly patients with type 2 diabetes visiting a community hospital were as sessed for health care problems.Results The patients had a total of 2 300 health care problems,mostly in health related behaviors (42.4％),and physiological domain(40.2％).Health care problems which had a incidence rate over 50.0％ lay in serum glucose monitoring(97.8％),medication regimen(95.6％),foot care(95.0％),nutrition(94.0％),circulation (92.8％),contacting comnunity resources(70.0 ％),visual acuity (68.3 ％),etc.Conclusion The elderly patients with type 2 diabetes have plenty health care problems in health related behaviors and physical fields.Nurses should formulate targeted care plans for patients timely and accurately according to the results,help them form correct health related behaviors,and encourage them to take part in social activities,in an attempt to reach physical,psychological,and social well being.
